补体3、免疫球蛋白G和胱抑素C与2型糖尿病患者肾功能的相关性分析

Correlation of Complement 3,Immunoglobulin G and Cystatin C with Renal Function in Type 2 Diabetic Patients

目的探讨补体3(C3)、免疫球蛋白G(IgG)和胱抑素C(CysL)与2型糖尿病患者肾功能之间的相关性。方法选取2016年6月至2018年6月在我院住院治疗的2型糖尿病患者130例,同时选取同期在我院进行健康体检的志愿者60名;按照肾功能的损害程度分为:肾功能轻度损害组(血尿素7.1~10.0 mmol/L,血肌酐<200μmol/L,n=71例)和肾功能重度损害组(血尿素>10.0 mmol/L,血肌酐>200μmol/L,n=59例)。通过监测Cys C、IgG、补体C3及肾功能各项指标,进一步分析C3、IgG和CysL与2型糖尿病患者肾功能的相关性。结果与健康对照组相比,患者组Urea、Cr及UA各项肾功能指标均有升高;且肾功能重度损害组比肾功能轻度损害组各项数值升高更明显,且差异有统计学意义(P<0.05);患者组Cys C、IgG和C3的检测值均有明显升高(P<0.05),且肾功能重度损害组比肾功能轻度损害组各项数值升高更明显,且差异有统计学意义(P<0.05);同时,通过Pearson相关性分析的结果提示,Cys C、IgG和C3与Urea、Cr、β2-Mg及UA各项肾功能指标间均有明显的正相关性(P<0.05)。结论与肾功能指标相比,Cys C、IgG和C3能更敏感地反映肾脏功能的变化,有助于2型糖尿病肾病的早期诊断,值得临床推广应用。

Objective To investigate the correlation of complement 3,immunoglobulin G and cystatin C with renal function in type 2 diabetic patients.Methods 130 patients with type 2 diabetes mellitus hospitalized in our hospital from June 2016 to June 2018 were selected,and 60 volunteers who underwent physical examination at the same time were selected.According to the degree of damage to renal function,they were divided into mild impairment group(blood urea 7.1~10.0 mmol/L,serum creatinine<200μmol/L;n=71 cases)and renal function severe impairment group(blood urea>10.0 mmol/L;serum creatinine>200μmol/L;n=59 cases).By monitoring the indexes of Cys C,IgG,C3 and renal function,the correlation of complement 3,immunoglobulin G and cystatin C with renal function in type 2 diabetic patients was further analyzed.Results Compared with the healthy control group,the Urea,Cr and UA indexes of renal function in the patient group were increased,and the values of Cys C,IgG and C3 in the severely impaired group were significantly higher than those in the mildly impaired group(P<0.05),and the values of Cys C,IgG and C3 in the patient group were significantly higher than those in the mild impaired group(P<0.05).Meanwhile,Pearson correlation analysis showed that Cys C,IgG and C3 were positively correlated with Urea,Cr,beta 2-Mg and UA(P<0.05).Conclusion Compared with renal function indexes,Cys C,IgG and C3 can reflect the changes of renal function more sensitively,which is helpful to the early diagnosis of type 2 diabetic nephropathy and worthy of clinical application.
