摘要
目的分析感染性肺炎新生儿血清可溶性细胞间黏附分子-1(sICAM1)、血清25羟维生素D[25(OH)D]水平与炎性因子的关系。方法采用前瞻性研究方法,选取咸阳市中心医院2017年8月至2019年8月期间收治的120例感染性肺炎新生儿为研究对象,其中49例重症患儿为重症组,71例轻症患儿为轻症组,另选同期健康新生儿50例为对照组。采用酶联免疫吸附试验法检测三组新生儿的血清sICAM1、25(OH)D及炎性因子水平,并进行组间对比,采用Pearson相关性分析血清sICAM1、25(OH)D与炎性因子水平的相关性。结果重症组患儿的血清sICAM1、肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)及降钙素原(PCT)分别为(261.38±34.68)ng/mL、(98.23±10.23)pg/L、(51.76±8.11)ng/L、(0.65±0.15)μg/L,明显高于轻症组的(211.09±28.34)ng/mL、(71.87±9.18)pg/L、(29.44±6.87)ng/L、(0.52±0.10)μg/L和对照组的(123.98±16.23)ng/mL,(43.23±6.87)pg/L、(12.86±4.03)ng/L、(0.29±0.05)μg/L,差异均有统计学意义(P<0.05);重症组患儿的血清25(OH)D水平为(10.89±3.01)μg/L,明显低于轻症组的(14.38±4.12)μg/L和对照组的(35.80±5.68)μg/L,差异均有统计学意义(P<0.05);经Pearson相关性分析结果显示,感染性肺炎新生儿血清sICAM1与TNF-α、IL-6、PCT均呈正相关(r=0.512、0.498、0.479,P<0.05),血清25(OH)D与TNF-α、IL-6、PCT均呈负相关(r=-0.487、-0.509、-0.490,P<0.05)。结论感染性肺炎新生儿血清sICAM1水平异常升高,25(OH)D水平异常降低,且血清sICAM1和25(OH)D与炎性因子均有相关性,监测新生儿血清sICAM1、25(OH)D水平可为新生儿感染性肺炎的诊断、病情评估提供一定参考。
Objective To analyze the relationship between serum soluble intercellular adhesion molecule-1(sICAM1),serum 25 hydroxyvitamin D[25(OH)D]and inflammatory factors in neonates with infectious pneumonia.Methods The prospective study was conducted on 120 newborn infants with pneumonia from August 2017 to August 2019 in Xianyang Central Hospital,including 49 severe children(the severe group)and 71 mild children(the mild group),and another 50 healthy newborns in the same period were selected as the control group.The levels of serum sICAM1,25(OH)D,and inflammatory factors in three groups were detected by enzyme-linked immunosorbent assay,and compared between groups.Pearson correlation analysis was used to explore the correlation between serum sICAM1,25(OH)D and disease condition and inflammatory factors.Results The serum levels of sICAM1,tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),and procalcitonin(PCT)in the severe group were(261.38±34.68)ng/mL,(98.23±10.23)pg/L,(51.76±8.11)ng/L,(0.65±0.15)μg/L,respectively,which were significantly higher than corresponding(211.09±28.34)ng/mL,(71.87±9.18)pg/L,(29.44±6.87)ng/L,(0.52±0.10)μg/L in the mild group and corresponding(123.98±16.23)ng/mL,(43.23±6.87)pg/L,(12.86±4.03)ng/L,(0.29±0.05)μg/L in the control group(all P<0.05);but the levels of serum 25(OH)D in the severe group was(10.89±3.01)μg/L,which was significantly lower than(14.38±4.12)μg/L in the mild group and(35.80±5.68)μg/L in the control group(P<0.05).The serum sICAM1 was positively correlated with TNF-α,IL-6 and PCT in neonates with septic pneumonia(r values=0.512,0.498,0.479,P<0.05),and serum 25(OH)D was negatively correlated with TNF-α,IL-6 and PCT(r values=-0.487,-0.509,-0.490,P<0.05).Conclusion The level of serum sICAM1 is abnormally increased and the level of 25(OH)D is abnormally decreased in neonates with infectious pneumonia,and both serum sICAM1 and 25(OH)D are correlated with inflammatory factors.Monitoring the serum sICAM1 and 25(OH)D levels in neonates can provide some reference for the diagnosis and disease assessment of neonatal infectious pneumonia.
