2D-TVS联合3D-TVS容积成像在子宫内膜病变诊断中的临床价值

Clinical value of two-dimension transvaginal sonography combined with three-dimension transvaginal sonography volumetric imaging in the diagnosis of endometrial lesions

目的探讨经阴道二维超声(2D-TVS)联合经阴道三维超声(3D-TVS)容积成像在子宫内膜病变诊断中的临床价值。方法选择性纳入2018年3月至2020年4月湖南省妇幼保健院收治且经病理确诊的462例子宫内膜病变患者,均进行2D-TVS、3D-TVS容积成像及联合检查。比较2D-TVS与3D-TVS容积成像的图像清晰度优良率;以病理检查结果为金标准,比较3种方式的诊断符合率。结果462例患者中良性内膜病变440例(95.24%),恶性内膜病变22例(4.76%);3D-TVS容积成像的图像清晰度优良率为95.02%,明显高于2D-TVS的69.26%,差异有统计学意义(P<0.05);联合检查及3D-TVS容积成像的诊断符合率分别为96.32%、89.18%,明显高于2D-TVS的71.43%,而联合检查的诊断符合率又明显高于3D-TVS容积成像,差异均有统计学意义(P<0.05)。结论2D-TVS联合3D-TVS容积成像在子宫内膜病变诊断中具有较高的临床价值。

Objective To investigate the clinical value of two-dimension transvaginal sonography(2D-TVS)combined with three-dimension transvaginal sonography(3D-TVS)volumetric imaging in the diagnosis of endometrial lesions.Methods A total of 462 patients with endometrial lesions diagnosed by pathology were selected in Hunan Maternal and Child Health Hospital from March 2018 to April 2020,and 2D-TVS and 3D-TVS volumetric imaging and combined examination were performed.The good rates of image sharpness were compared between 2D-TVS and 3D-TVS volumetric imaging.The results of pathological examination were used as the gold standard to compare the diagnostic coincidence rates between the three methods.Results Among the 462 patients,440(95.24%)had benign endometrial lesions and 22(4.76%)had malignant endometrial lesions.The good rates of image sharpness of 3D-TVS volumetric imaging were 95.02%,which were significantly higher than 69.26%of 2D-TVS(P<0.05).The diagnostic coincidence rates of combined examination and 3D-TVS volumetric imaging were 96.32%and 89.18%,respectively,which were significantly higher than 71.43%of 2D-TVS,and the diagnostic coincidence rates of combined examination were significantly higher than that of 3D-TVS volumetric imaging,with statistically significant differences(P<0.05).Conclusion 2D-TVS combined with 3D-TVS volumetric imaging had high clinical value in the diagnosis of endometrial lesions.
