磁共振成像对胎儿神经系统先天发育异常的临床诊断价值研究 被引量:4

摘要 目的探讨磁共振成像对胎儿神经系统先天发育异常的临床诊断价值。方法选取我院收治的疑似胎儿神经系统发育异常的孕妇130例,所有孕妇均进行超声和磁共振成像检查,然后对胎儿不同部位神经系统发育异常的MRI影像特点进行分析,并对超声和MRI检查神经系统发育异常的图像质量和诊断价值进行分析。结果超声对胎儿神经系统发育异常检查的脉络丛室管膜显示和血流显示的评分高于MRI检查,而MRI检查的视野、分辨率、脑沟、裂、回显示和组织特异性评分高于超声检查,差异具有统计学意义(P<0.05);MRI对胎儿神经系统发育异常诊断的灵敏度、特异度和正确指数均高于超声检查,且阳性预测值和阴性预测值也高于超声检查。结论磁共振成像对胎儿神经系统发育异常诊断的准确率较超声高,误诊率低,提供的影像学资料更全面和准确,值得在临床上推广使用。 Objective To explore the clinical diagnostic value of magnetic resonance imaging in congenital dysplasia of fetal nervous system.Methods 130 pregnant women who were suspected of congenital dysplasia of fetal nervous system were collected from our hospital,ultrasound and magnetic resonance imagings were performed in all pregnant women,then the MRI features of the different parts of the fetal nervous system dysplasia were analyzed,and the image quality and diagnostic value of ultrasonography and MRI in the diagnosis of nervous system dysplasia were also analyzed.Results The score of choroid plexus ependymography and blood flow display by ultrasonography was higher than that by MRI,while the scoring of visual field,resolution,sulcus,fissure,echo and tissue specificity by MRI was higher than that by ultrasound,the differences were statistically significant(P<0.05).The sensitivity,specificity and correct index of MRI in the diagnosis of fetal nervous system dysplasia were higher than those of ultrasound,and the positive predictive value and negative predictive value were higher than those of ultrasound.Conclusion The accuracy of magnetic resonance imaging in the diagnosis of fetal nervous system dysplasia is higher than that of ultrasound,and the misdiagnosis rate is lower,the imaging data provided are more comprehensive and accurate,thereby recommending in clinic.
