益气养阴通络法治疗类风湿关节炎缓解期经验

Experience on Treating Rheumatoid Arthritis in Remission Stage by Method of Supplementing Qi,Nourishing Yin and Dredging Collaterals

高明利教授认为类风湿关节炎缓解期主要表现为气阴两虚证,基本病机为本虚标实,本虚以气阴两虚为主,标实以湿瘀阻络为要。提出益气养阴通络法治疗类风湿关节炎缓解期,并基于古方四神煎加减化裁拟益气养阴通络方发挥益气养阴、通络止痛之功,既可扶正,又能祛邪,补而不滞,清而不寒。应用上,善加用藤类、虫类药物,可入络通脉,祛邪止痹;又恐邪实盛重,正气不足而加重病情,故重视脾胃顾养,以充正气抗邪之力;又根据发病部位选用引经药,引药直达病所,切中要害,疗效显著。

Professor GAO Mingli holds that the major manifestation of rheumatoid arthritis in remission stage is deficiency of qi and yin,and the primary disease mechanism is deficient root and excess branch.Deficient root refers to deficiency of qi and yin,and excess branch refers to dampness and blood stasis blocking meridians.Therefore,replenishing qi,nourishing yin and dredging collaterals should be applied to treat rheumatoid arthritis in remission stage.A formulas for supplementing qi,nourishing yin and dredging collaterals has been invented based on classical formula Sishen Decoction(四神煎),with the function of supplementing qi and nourishing yin,dredging collaterals and relieving pain.Generally speaking,this formula can not only strengthen healthy qi,but also dispel evil qi.It could tonify without stagnation,and clear without coldness.In clinical application,Professor GAO is good at using rattan and insect medicinals,for they can enter the collaterals and dredge the vessels,and dispel evil qi and relieve arthralgia.During the treatment,to avoid worsening of disease condition due to insufficient healthy qi,he attaches importance to nourishing the spleen and stomach to promote healthy qi in resisting evil qi.Moreover,according to the location of the disease,He uses tropism medicianls to direct medicinals to reach the disease location and attack directly,and the effect is good.
