信息化优先定向转运在救治急性STEMI患者中的作用 被引量:1

摘要 目的:探讨应用信息化优先定向转运对急性ST段抬高型心肌梗死(STEMI)患者救治疗效的影响。方法:本文回顾性分析2018年1月至2019年12月经增城区120指挥中心调度转运至区人民医院胸痛中心救治并行急诊经皮冠状动脉介入(PCI)治疗的急性STEMI患者临床资料,随机抽取应用信息化优先定向转运之前救治的急性STEMI患者90例(2018年1月至2018年12月)为对照组,随机抽取应用信息化优先定向转运之后救治的急性STEMI患者90例(2019年1日至2019年12月)为观察组。比较两组首次医疗接触至球囊扩张的时间(FMC-to-B)、进门-球囊时间(D-to-B)、急救成功率、院内一周病死率、心力衰竭发生率、平均住院天数和住院费用。结果:与对照组比较,观察组可缩短急性STEM患者FMC-to-B、D-to-B、提高急救成功率、降低院内一周病死率、心力衰竭发生率、住院天数及费用,两组差异均有统计学意义(P<0.05)。结论:信息化优先定向转运救治急性STEMI患者的应用,可明显缩短其FMC-to-B、D-to-B,提高急救成功率,减少住院天数及费用,具有良好的社会效益和经济效益,建议推广应用。 Objective:To explore the effect of the application of information-based preferential directional transport on the treatment effectof patients with acute ST-segment elevation myocardial infaretion(STEMI). Methods:This article retrospectively analyzes the clinical data of acute STEMI patients who were transferred to the chest pain center of the District People's Hospital from January 1,2018 to December 31,2019 through the district 120 command center for treatment and emergency percutaneous coronary intervention(PCI)treatment,and the application of information-based priority directional transportation was randomly selected. 90 patients with acute STEMI were previously treated from Jan2018 to Dec 2018 were classified as the control group,and 90 patients with acute STEMI were treated after the application of information-based priority directional transfer were randomly selected from Jan 2019 to Dec 2019 as the observation group. The times of first medical contact to balloon(FMC-to-B)and door to balloon(D-to-B),success rate,fatality rate during one week of hospitalization,the incidence of heart failure,the average length of stay in hospital and the cost of hospitalization were compared between the two groups. Results:Compared with the control group,the time of FMC-toB and D-to-B shorted,the success rate of first aid increased,while the hospitalization period,the one-week fatality rate,the incidence of heart failure,the length of hospitalization and the cost reduced,all the difference between the two groups were statistically significant(P<0.05). Conclusion:The application of information-based priority directional transport to treat patients with acute STEMI can greatly shorten the time of FMC-to-B and(D-to-B of patients with acute STEM. The application has good social and economic benefits and is worthy of promotion because it can improve the success rate of first aid,reduce the number of days and costs of hospitalization.
