氢质子磁共振波谱与PET/CT对颞叶癫痫致痫灶定位诊断的对比研究 被引量:4

摘要 目的:探讨^(1)H-MRS、PET/CT对颞叶癫痫患者致痫灶定位的诊断价值。方法:回顾性分析我院2017年1月—2020年6月经病理证实的60例难治性颞叶癫痫的影像学资料,比较术前18F-FDG PET/CT、^(1)H-MRS检查及联合两种检查技术对颞叶癫痫患者致痫灶定侧的准确率。结果:60例颞叶癫痫患者中,^(1)H-MRS定侧准确率为58.3%,PET/CT定侧准确率为85.0%(51/60),两种联合诊断正确率90.0%(54/60),χ^(2)=20.121,P<0.01,差异有统计学意义。结论:PET/CT对颞叶癫痫患者致痫灶定侧的准确率高于^(1)H-MRS,联合两者检查手段提高了颞叶癫痫致痫灶定侧准确率。 Objective To investigate the diagnostic value of 1H-MRS and PET/CT in the localization of epileptic foci in patients with temporal lobe epilepsy.Methods The imaging data of 60 cases of intractability temporal lobe epilepsy confirmed by pathology in our hospital from January 2017 to June 2020 were retrospectively analyzed,and the accuracy of preoperative PET/CT,1H-MRS examination and the combination of the two examination techniques in the lateralization of epileptic foci in patients with temporal lobe epilepsy was compared.Results In 60 patients with temporal lobe epilepsy,the accuracy of 1H-MRS and PET/CT was 58.3%(35/60),85.0%(51/60),and 90.0%(54/60).The chi-square test showed that the chi-square value was 20.121(P<0.01),and the difference was statistically significant.Conclusion The accuracy of PET/CT in the localization of epileptic foci in patients with temporal lobe epilepsy is higher than that of 1H-MRS,and the combination of PET/CT and 1H-MRS can improve the accuracy of the localization of epileptic foci in temporal lobe epilepsy.
