摘要
目的评价成人安氏Ⅱ类2分类错[牙合]畸形患者正畸排齐整平后的咬合特征。方法从2017年1月至2019年6月于郑州大学第一附属医院正畸科就诊的患者中选取20例安氏Ⅱ类2分类错[牙合]畸形成人患者,应用T-ScanⅢ系统对其进行咬合检查,记录排齐整平前后在最大牙尖交错位(MIP)、前伸运动及侧方运动的[牙合]力中心点位置(COF)、咬合接触时间(OT)和分离时间(DT)、[牙合]力百分比及咬合接触点数目。结果排齐整平后,MIP OT、COF和前伸运动DT均较排齐整平前缩短(P<0.05)。排齐整平后,MIPⅠ类侧切牙区[牙合]力百分比较排齐整平前降低,磨牙区[牙合]力百分比较排齐整平前升高,Ⅱ类侧切牙区、尖牙及前磨牙区[牙合]力百分比均较排齐整平前降低;侧方运动工作侧[牙合]力百分比均较排齐整平前升高,非工作侧[牙合]力百分比均较排齐整平前降低(P<0.05)。排齐整平后,MIPⅠ类侧切牙区、尖牙及前磨牙区咬合接触点均较排齐整平前减少,Ⅱ类侧切牙区咬合接触点较排齐整平前减少,磨牙区咬合接触点较排齐整平前增加;前伸运动Ⅰ类侧切牙区咬合接触点较排齐整平前增加;Ⅰ类侧侧方运动工作侧咬合接触点较排齐整平前增加,非工作侧咬合接触点较排齐整平前减少(P<0.05)。结论正畸排齐整平有助于调整安氏Ⅱ类2分类错[牙合]畸形成人患者的咬合接触关系,从而使全口咬合趋于协调稳定,但后期仍需治疗。
Objective To evaluate the occlusal characteristics in adult patients with classⅡdivision 2 subdivision malocclusion after leveling and aligning.Methods A total of 20 adult patients with classⅡdivision 2 subdivision malocclusion were selected from the Department of Orthodontic of the First Affiliated Hospital of Zhengzhou University from January 2017 to June 2019.The occlusal examination was performed with T-ScanⅢsystem,and the center of force(COF),occlusal time(OT),disclusion time(DT),percentage of occlusal force and number of occlusal contact points were recorded at the maximum intercuspal position(MIP),anterior extension movement and lateral movement before and after leveling and aligning.Results After leveling and aligning,the OT、COF at MIP and DT at anterior extension movement were all shorter than those before alignment and leveling(P<0.05).Compared with those before leveling and aligning,after leveling and aligning,the percentage of occlusal force of classⅠ-sided incisors at MIP decreased,and the percentage of occlusal force of classⅠ-sided molars at MIP increased,and the percentages of occlusal force of classⅡ-sided incisors,canines and premolars at MIP decreased,and the percentages of occlusal force of working sides at lateral movement increased,while the percentages of occlusal force of non-working sides decreased(P<0.05).Compared with those before leveling and aligning,after leveling and aligning,the occlusal contact points of classⅠ-sided incisors,canines and premolars at MIP reduced,and the occlusal contact point of classⅡ-sided incisors at MIP reduced,and the occlusal contact points of molars at MIP increased,and the occlusal contact point of classⅠ-sided incisors at anterior extension movement increased,and the occlusal contact point of working side at classⅠ-sided lateral movement increased,while the occlusal contact point of non-working side decreased(P<0.05).Conclusion The occlusal contact relationship could be improved by orthodontic leveling and aligning in adult patients with classⅡdivision 2 subdivision malocclusion,and the bite is tended to be balanced and stable.However,further treatment is still needed.
