不同肠内营养支持策略对重型颅脑损伤患者预后影响

To Investigate the Clinical Application of Different Enteral Nutrition Support Strategiesin Patients with Severe Head Injury

目的:探讨不同肠内营养支持策略对重型颅脑损伤患者预后影响.方法:收集2019年6月-2020年6月在武汉科技大学附属天佑医院神经外科住院的40例重型颅脑损伤患者为研究对象,随机分为对照组(单纯肠内营养组20例)及观察组(肠内营养添加ω-3多不饱和脂肪酸组20例),比较2组1、3、7、14天血清白蛋白、前白蛋白水平;并比较2组治疗3个月时格拉斯哥预后评分(Glasgow outcome score,GOS).结果:2组患者血清白蛋白、前白蛋白水平经治疗后均有所上升,第14天与第1天比较,差异有统计学意义(P<0.05),组间比较差异无统计学意义(P>0.05).随访3个月时,观察组GOS评分优于对照组,差异具有统计学意义(P<0.05).结论:肠内营养如果联合ω-3多不饱和脂肪酸用于重型颅脑损伤的治疗,可以改善患者的营养状况及预后.

Objective:To investigate the clinical application of different enteral nutrition support strategies in patients with severe head injury.Methods:A total of 40 patients with severe craniocerebral injury who were hospitalized in the Department of Neurosurgery,Tianyou Hospital,Wuhan University of Science and Technology from June 2019 to June 2020 were randomly selected into the control group(without the ω-3 polyunsaturated fatty acid group 20 Example)and experimental group(20 cases added with ω-3 polyunsaturated fatty acid group),comparing serum albumin,prealbumin levels at 1,3,5 and 7 days after treatment,and GOS scores were evaluated after treatment lasted for 3 months.Results:Serum albumin and prealbumin levels in the two groups increased after treatment.The difference between day 14 and day 1 was statistically significant(P<0.05),and there was no significant difference between the groups(P>0.05).on the 3 months,the GOS score of the test group was better than the control group,and the difference was statistically significant(P<0.05).Conclusion:If enteral nutrition combined with ω-3 polyunsaturated fatty acids is used in the treatment of severe craniocerebral injury,it can improve the nutritional status and reduce the inflammatory response of patients,which may have a positive effect on improving the prognosis of patients.
