中药熏洗联合穴位敷贴治疗对肛肠疾病术后疼痛的效果和安全性研究

Study on effect and safety of Chinese herbal fumigation combined with acupoint application on postoperative pain of anorectal disease

目的探讨中药熏洗联合穴位敷贴治疗对肛肠疾病术后疼痛的效果和安全性。方法60例肛肠疾病术后疼痛患者,随机分为观察组和对照组,各30例。对照组给予穴位敷贴治疗,观察组在对照组基础上联合中药熏洗治疗。比较两组治疗前后视觉模拟评分法(VAS)评分及换药后疼痛持续时间、创面愈合时间、临床疗效、不良反应发生情况。结果术后第1天,观察组和对照组换药后疼痛持续时间分别为(3.26±0.72)、(3.14±0.69)h,两组比较差异无统计学意义(P>0.05);术后第3、5、7天,观察组换药后疼痛持续时间分别为(2.39±0.67)、(1.87±0.54)、(1.24±0.36)h均明显短于对照组的(2.87±0.59)、(2.31±0.61)、(1.72±0.43)h,差异有统计学意义(P<0.05)。观察组创面愈合时间为(13.06±4.26)d,短于对照组的(20.65±5.37)d,差异有统计学意义(P<0.05)。治疗前,两组VAS评分比较,差异无统计学意义(P>0.05);治疗后,观察组和对照组VAS评分分别为(3.83±0.86)、(4.57±0.98)分均显著低于治疗前的(7.63±1.22)、(7.54±1.06)分,且观察组低于对照组,差异有统计学意义(P<0.05)。观察组总有效率为93.33%,高于对照组的73.33%,差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论对肛肠疾病术后疼痛患者进行中药熏洗联合穴位敷贴治疗可明显改善疼痛情况,提高治疗效果,疼痛持续时间明显缩短,加速创面愈合,并且具有较高安全性,具有临床推广应用价值。

Objective To discuss the effect and safety of Chinese herbal fumigation combined with acupoint application on postoperative pain of anorectal disease.Methods A total of 60 patients with postoperative pain of anorectal disease were randomly divided into observation group and control group,with 30 cases in each group.The control group was treated with acupoint application,and the observation group was treated with Chinese herbal fumigation on the basis of the control group.The visual analogue scale(VAS)score,duration of pain after dressing change,wound healing time,clinical efficacy,and occurrence of adverse reactions were compared between the two groups.Results 1 d after operation,the pain after dressing change of the observation group and control group were(3.26±0.72)and(3.14±0.69)h,and the difference was not statistically significant(P>0.05).3,5 and 7 days after operation,the duration of pain after dressing change of the observation group were(2.39±0.67),(1.87±0.54)and(1.24±0.36)h,which were obviously shorter than(2.87±0.59),(2.31±0.61)and(1.72±0.43)h of the control group,and the difference was statistically significant(P<0.05).The wound healing time(13.06±4.26)d of the observation group was shorter than(20.65±5.37)d of the control group,and the difference was statistically significant(P<0.05).Before treatment,there was no statistically significant difference in VAS score between the two groups(P>0.05).After treatment,the VAS score of the observation group and control group were(3.83±0.86)and(4.57±0.98)points,which were significantly lower than(7.63±1.22)and(7.54±1.06)points before treatment;the observation group was lower than the control group.All the difference was statistically significant(P<0.05).The total effective rate 93.33%of the observation group was higher than 73.33%of the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in incidence of adverse reactions between the two groups(P>0.05).Conclusion Combination of Chinese herbal fumigation and acupoint application can obviously improve the pain status of patients with postoperative pain of anorectal disease,which can improve the therapeutic effect,obviously shorten duration of pain,accelerate wound healing with high safety.It has clinical promotion and application value.
