一次性宫腔压迫球囊治疗产后出血产妇的应用价值

Practical value of disposable intrauterine compression balloon in the treatment of postpartumhemorrhage

目的研究产后出血产妇使用一次性宫腔压迫球囊治疗的效果。方法82例产后出血产妇作为研究对象,随机分为研究组和对照组,各41例。对照组阴道分娩产妇止血方式为宫腔内纱布填塞法,剖宫产产妇行B-Lynch子宫缝合术;研究组阴道分娩及剖宫产产妇均行一次性宫腔压迫球囊治疗。对比两组产妇出血情况及输血量、母体结局(止血率、子宫切除率、子宫动脉结扎率)。结果研究组产妇术中出血量、术后2 h出血量、术后24 h出血量及输血量均明显少于对照组,差异具有统计学意义(P<0.05)。对照组产妇止血率为82.9%(34/41)、子宫动脉结扎率为17.1%(7/41)、子宫切除率为12.2%(5/41),研究组产妇止血率为97.6%(40/41)、子宫动脉结扎率为2.4%(1/41)、子宫切除率为0(0/41);研究组产妇止血率高于对照组,子宫动脉结扎率、子宫切除率均低于对照组,差异均具有统计意义(P<0.05)。结论通过对产后出血产妇使用一次性宫腔压迫球囊治疗,可有效减少产妇出血量和输血量,并改善母体结局,应用效果理想。

Objective To study the effect of disposable intrauterine compression balloon in the treatment of postpartum hemorrhage.Methods A total of 82 cases of postpartum hemorrhage as study subjects were randomly divided into research group and control group,with 41 cases in each group.In the control group,intrauterine gauze packing and B-Lynch uterine suture were used for vaginal delivery and cesarean section cases,respectively;in the research group,B-Lynch uterine suture was were used for vaginal delivery and cesarean section cases. The blood loss, blood transfusion volume and maternal outcomes (hemostasis rate, hysterectomyrate, uterine artery ligation rate) were compared between the two groups. Results The intraoperative blood loss,2-h postoperative blood loss, 24-h postoperative blood loss and blood transfusion volume of the research groupwere obviously less than those of the control group, and the difference was statistically significant (P<0.05).The hemostasis rate, uterine artery ligation rate, hysterectomy rate of the control group were 82.9%(34/41),17.1%(7/41), 12.2%(1/41), which were 97.6%(40/41), 2.4%(1/41), 0(0/41) of the research group. The hemostasisrate of the research group was higher than that of the control group, and uterine artery ligation rate andhysterectomy rate were lower than those of the control group. All the difference was statistically significant (P<0.05).Conclusion Disposable intrauterine compression balloon shows ideal effect in the treatment of postpartumhemorrhage, which can effectively reduce blood loss and blood transfusion volume, and improve the maternaloutcome.
