摘要
冠状动脉疾病的治疗方案和预后评估与冠状动脉微循环结构和功能密切相关。冠状动脉微循环功能障碍(CMD)的程度对于心肌梗死患者选择最佳的治疗方案和恰当的术后管理具有重要意义。有创性定量评估可以准确评估CMD,但其为有创检查,专业性强且操作复杂,在心血管疾病的诊断与检测方面有一定局限性。而无创性定量评估心肌缺血能客观、准确地早期诊断和评估冠状动脉微循环功能,且可以结合其他临床数据,改善心血管疾病的危险分层和优化个体化治疗评估。未来,可将无创性定量评估心肌缺血研究与机器学习相结合,以有利于临床寻找更加科学合理的治疗方案。
The treatment plan and prognosis evaluation of coronary artery disease are closely related to the structure and function of coronary microcirculation. The extent of coronary microcirculation dysfunction( CMD) is important for the selection of optimal treatment and appropriate postoperative management for patients with myocardial infarction. Invasive quantitative assessment can accurately assess CMD,but it is invasive,highly specialized and complex,and has certain limitations in the diagnosis and detection of cardiovascular diseases. Noninvasive quantitative evaluation of myocardial ischemia can objectively and accurately diagnose and evaluate coronary microcirculation function in an early stage,and can improve the risk stratification of cardiovascular disease and optimize individualized treatment evaluation in combination with other clinical data. In the future,noninvasive quantitative assessment of myocardial ischemia can be combined with machine learning,which is conducive to the clinical search for more scientific and reasonable treatment.
