^(18)F-FDG PET/CT对偶发性SPN独立影响因素再评估

摘要 目的探讨偶发性孤立性肺结节(SPN)的临床特点及在^(18)F-FDG PET/CT影像诊断中的特征。方法回顾性分析2016年1月至2017年12月行PET/CT发现的100例孤立性肺结节患者,分析相关病史及^(18)F-FDG PET/CT影像学资料。结果100例患者中,确诊恶性肿瘤65例,良性病变21例,假阳性6例,假阴性8例;均经病理学诊断或临床随访超过2年。分析SPN诸多影响因素中,患者性别、吸烟史、肿瘤史、直径大小、结节部位、空泡征、血管集束、胸膜牵拉、支气管截断、最大标准化摄取值(SUVmax)等因素关联对良恶性统计学无明显差异。单因素分析结果显示,患者年龄、分叶征、毛刺征在良恶性鉴别诊断中有统计学意义(P<0.05)。结论^(18)F-FDG PET/CT对判断偶发性SPN良恶性鉴别有重要价值,患者年龄、分叶征、毛刺征与孤立性肺结节良恶性有关。 Objective To investigate the clinical and imaging festures of ^(18)F-FDG PET/CT examination in diagnosis of solitary pulmonary nodules(SPN).Methods The clinical data about 100 patients with SPN who were diagnosed by PET/CT in our hospital from January 2016 to October 2017 were analyzed retrospectively.The medical history and imaging data of ^(18)F-FDG PET/CT were analyzed.Results Among the 100 patients,there were 65 cases of malignant tumors,21 cases of benign lesions,6 cases were false positive,8 cases were false negative.All the patients were diagnosed pathologically or followed up clinically for more than 2 years.In the influencing factors on SPN,there were no significant differences in patient’s gender,smoking history,tumor history,diameter,nodule location,vacuolar sign,vascular bundle,pleural traction,bronchial truncation,maximum standardized uptake value(SUVmax)beween benign tumors and malignant ones.Univariate analysis showed that there were significant differences in patient’s age,foliar sign and burr sign in differential diagnosis beween benign tumors and malignant ones(P<0.05).Conclusion The ^(18)F-FDG PET/CT has important significance in differential diagnosis beween benign tumors and malignant ones,and patient’s age,foliar sign and burr sign are related with benign SPN or malignant SPN.
