加味归肾丸治疗薄型子宫内膜肾虚血瘀证50例临床观察

Clinical observation on 50 cases of thin endometrium with kidney deficiency and blood stasis syndrome treated by Jiawei Guishen Wan(加味归肾丸)

目的观察加味归肾丸治疗薄型子宫内膜肾虚血瘀证的临床疗效。方法将100例薄型子宫内膜肾虚血瘀证患者采用随机数字表法分为对照组和治疗组,各50例。对照组给予补佳乐和黄体酮胶囊治疗,治疗组给予加味归肾丸治疗,2组均连续治疗3个月。比较2组的总有效率、妊娠率,以及治疗前后排卵日子宫内膜厚度,卵泡直径,黄体期血清雌二醇(E_(2))、孕酮(P)水平。结果治疗后2组排卵日子宫内膜厚度及卵泡直径均明显增大,血清E_(2)、P水平均明显升高,与同组治疗前比较差异有统计学意义(P<0.05),且治疗组对各项指标的改善作用明显优于对照组,2组比较差异均有统计学意义(P<0.05);治疗组妊娠率和总有效率分别为75.0%(15/20),90%(45/50),对照组分别为61.1%(11/18),74%(37/50),2组比较差异均有统计学意义(P<0.05)。结论加味归肾丸可明显改善薄型子宫内膜肾虚血瘀证患者的排卵日子宫内膜厚度、卵泡直径及黄体期血清E_(2)、P水平,提高妊娠率,值得临床推广应用。

Objective To observe the clinical efficacy of Jiawei Guishen Wan( 加味归肾丸) in the treatment of thin endometrium with kidney deficiency and blood stasis syndrome. Methods Totally 100 cases of thin endometrium with kidney deficiency and blood stasis syndrome were divided into treatment group and control group according to random number table method,with 50 cases in each group. The control group was given estradiol valerate tablets and progesterone capsules,while the treatment group was given modified Jiawei Guishen Wan,both groups were consecutively treated for 3 months. The total effective rate,pregnancy rate,endometrial thickness and follicle diameter on ovulation date and serum estradiol( E_(2)) and progesterone( P) levels in luteal phase before and after treatment were compared between the 2 groups. Results After treatment,the endometrial thickness and follicle diameter on ovulation date of the 2 groups were significantly increased,the serum E_(2) and P levels were significantly increased,and the difference was statistically significant compared with the same group before treatment( P<0.05),and the improvement effect on each index in the treatment group was significantly better than that in the control group,and the difference was statistically significant( P < 0.05). The pregnancy rate and total effective rate of the treatment group were 75.0%( 15/20) and 90%( 45/50),respectively;and those of the control group were 61.1%( 11/18) and 74%( 37/50),respectively. The differences between the 2 groups were statistically significant( P<0.05).Conclusion Jiawei Guishen Wan can markedly improve the endometrial thickness and follicle diameter on ovulation date and luteal phase serum E_(2),P levels in patients of thin endometrium with kidney deficiency and blood stasis syndrome,and improve pregnancy rate,which is worthy of clinical promotion and application.
