MSCT与Gd-EOB-DTPA DCE-MRI在肝脏局灶性病变诊断中的价值

Value of MSCT and Gd-EOB-DTPA DCE-MRI in the Diagnosis of Focal Liver Lesions

目的:研究MSCT与钆塞酸二钠动态对比增强磁共振成像(Gd-EOB-DTPA DCE-MRI)在肝脏局灶性病变诊断中的价值。方法:回顾性选取笔者所在医院收治的疑似肝脏局灶性病变患者75例,选例时间为2019年2月-2020年10月,所有患者均进行Gd-EOB-DTPA DCE-MRI、MSCT的统一检查,比较手术病理结果与Gd-EOB-DTPA DCE-MRI、MSCT的检查结果,分析两种检查方式对肝脏局灶性病变的诊断价值。结果:在202枚病灶中,MSCT共诊断准确175枚,准确率为86.63%,诊断准确的恶性病灶144枚(92.31%),诊断准确良性病灶31枚(75.61%);Gd-EOB-DTPA DCE-MRI共诊断准确193枚,准确率为95.54%,诊断准确恶性病灶154枚(98.72%),诊断准确良性病灶39枚(95.12%);Gd-EOB-DTPA DCE-MRI的诊断价值(阳性预测值、阴性预测值、灵敏度、特异度、准确率)均高于MSCT检查(P<0.05)。结论:临床上相比于MSCT检查,Gd-EOB-DTPADCE-MRI亦可辅助诊断肝脏局灶性病变,且其诊断价值更高,应用效果较好。

Objective:To study the value of MSCT and Dynamic contrast-enhanced magnetic resonance imaging with gadolinium selenate (Gd-EOB-DTPA DCE-MRI) in the diagnosis of focal liver lesions.Method:A total of 75 patients with suspected focal liver lesions admitted to our hospital from February 2019 to October 2020 were retrospectively analyzed.MSCT and Gd-EOB-DTPA DCE-MRI were performed in all patients.The diagnostic efficacy of MSCT and Gd-EOB-DTPA DCE-MRI in focal liver lesions was compared with surgical pathological results.Result:Among 202 lesions,144 (92.31%) were malignant lesions,31 (75.61%) were benign lesions,and 175 (86.63%) were accurately diagnosed by MSCT;154 (98.72%) were malignant lesions,39 (95.12%) were benign lesions,and 193 (95.54%) were accurately diagnosed by GD-EOB-DTPA DCE-MRI.The diagnosis accuracy of GD-EOB-DTPA DCE-MRI in sensitivity,specificity,positive predictive value,negative predictive value and accuracy of DCE-MRI were higher than those of MSCT (P<0.05).Conclusion:Both MSCT and Gd-EOB-DTPA DCE-MRI are helpful in the diagnosis of focal liver lesions,but GD-EOB-DTPA DCE-MRI is more valuable in the diagnosis of focal liver lesions.
