以自我效能为导向的护理干预对经皮椎间孔镜腰椎髓核摘除术后患者康复效果影响

The Effect of Self-efficacy-oriented Nursing Intervention on the Rehabilitation Effect of Patients after Percutaneous Foraminal Resection of Lumbar Nucleus Pulposus

目的:探究以自我效能为导向的护理干预对经皮椎间孔镜腰椎髓核摘除术后患者康复效果影响。方法:择取2018年11月-2020年6月在钟祥市人民医院就诊的178例腰椎间盘突出症(LDH)患者,依据不同护理方案分为研究组与对照组,每组89例。对照组采用骨科常规护理方案,研究组则实施以自我效能为导向的护理方案,对比两组护理前后自我效能(GSES)评分、腰椎功能(JOA)评分、术后康复效果及并发症情况。结果:护理后1周、2个月,研究组GSES评分、JOA评分均高于对照组,差异均有统计学意义(P<0.05);研究组康复总有效率(98.88%)高于对照组(89.89%),并发症发生率(2.24%)低于对照组(11.24%),差异均有统计学意义(P<0.05)。结论:腰椎间盘突出症患者采用经皮椎间孔镜腰椎髓核摘除术治疗的围术期实施以自我效能为导向的护理方案,术后康复效果显著,且并发症发生低,可显著改善腰椎功能,提升患者自我效能感,值得临床推广应用。

Objective:To explore the effect of self-efficacy-oriented nursing intervention on the rehabilitation effect of patients after percutaneous intervertebral foramina resection of lumbar nucleus pulposus.Method:A total of 178 patients with lumbar disc herniation (LDH) admitted to Zhongxiang People’s Hospital from November 2018 to June 2020 were selected and divided into the study group and the control group according to different nursing plans,with 89 patients in each group.The control group received orthopedic routine nursing plan,while the research group received self-efficacy oriented nursing plan.The self-efficacy (GSES) score,lumbar function (JOA) score before and after nursing,postoperative rehabilitation effect and complications were compared between the two groups.Result:At 1 week and 2 months after nursing,GSES score and JOA score in the research group were higher than those in the control group,the differences were statistically significant (P<0.05).The total effective rate in the research group (98.88%) was higher than that in the control group (89.89%),and the incidence of complications (2.24%) was lower than that in the control group (11.24%),the differences were statistically significant (P<0.05).Conclusion:For patients with lumbar disc herniation treated by percutaneous foraminal resection of lumbar nucleus pulposus,the perioperative nursing program guided by self-efficacy has a significant postoperative rehabilitation effect,and the incidence of complications is low,which can significantly improve lumbar function and enhance patients’ sense of self-efficacy,and is worthy of clinical promotion and application.
