跨学科管理对甲状腺癌手术患者负性情绪、术后恢复的影响

Effect of interdisciplinary management on negative emotion and postoperative recovery of patients with thyroid cancer surgery

目的研究跨学科管理模式对甲状腺癌手术患者负性情绪及术后恢复的影响。方法选取外科行甲状腺癌手术切除的患者62例,按干预方法不同分为对照组与观察组,每组31例。对照组行常规管理方法,观察组行常规管理+跨学科管理。比较两组干预前后的焦虑自评量表(SAS)评分、抑郁自评量表(SDS)评分及生活质量核心量表(QLQ-C30)评分,比较两组患者干预后的视觉模拟评分法(VAS)评分并记录两组患者术后下床活动时间、住院时间及并发症发生情况。结果干预后,观察组患者的SAS评分、SDS评分、VAS评分均低于对照组,观察组患者的术后下床活动时间及住院时间均短于对照组,观察组患者的并发症总发生率(3.23%)低于对照组(19.35%),观察组患者QLQ-C30评分中躯体功能、情绪功能、角色功能、社会功能、认知功能及总健康状况评分均高于对照组,差异均有统计学意义(P﹤0.05)。结论跨学科管理能有效缓解甲状腺癌手术患者负性情绪,促进术后恢复,降低并发症发生率,提高患者生活质量。

Objective To investigate the effect of interdisciplinary management model on negative emotion and postoperative recovery of patients with thyroid cancer surgery.Method A total of 62 patients with thyroid cancer who underwent thyroidectomy were selected and divided into control group and observation group according to different intervention methods,with 31 cases in each group.The control group performed conventional management methods,and the observation group performed conventional management plus interdisciplinary management.The self-rating anxiety scale(SAS),self-rating depression scale(SDS)and quality of life questionnaire core 30(QLQ-C30)before and after the intervention were compared between the two groups.The visual analogue scale(VAS)score after the intervention was compared between the two groups,the postoperative ambulation time,hospitalization time and complications of two groups were recorded.Result After the intervention,the scores of SAS,SDS,and VAS of the observation group were significantly lower than those of the control group;the postoperative time of getting out of bed and hospital stay of the observation group were significantly shorter than those of the control group;the complication rate of the observation group was3.23%significantly lower than that of the control group with 19.35%;the scores of QLQ-C30 in physical function,emotional function,role function,social function,cognitive function,and general health status in the observation group were significantly higher than those of the control group,the differences were statistically significant(P<0.05).Conclusion Interdisciplinary management could effectively alleviate the negative emotion of thyroid cancer patients after surgery,promote postoperative recovery,reduce the incidence of complications and improve patients’quality of life.
