自身免疫性脑炎患儿的脑电活动与行为神经发育的相关性

Correlation between Brain Electrical Activity and Behavioral Neurodevelopment in Patients with Autoimmune Encephalitis

摘要 目的:探讨自身免疫性脑炎患儿的脑电活动与行为神经发育的相关性。方法:选择2015年1月—2020年6月本院收治的自身免疫性脑炎患儿85例作为脑炎组,同期选择在本院体检的健康小儿85例作为对照组,记录两组的振幅整合脑电图背景特征,评定小儿的行为神经发育情况并进行相关性分析。结果:脑炎组中CNV 25例,DNV 31例,BS 4例,CLV 25例,FT 0例;对照组中CNV 83例,DNV 2例,两组振幅整合脑电图背景活动分级对比差异有统计学意义(P<0.05)。脑炎组的神经发育评分显著低于对照组(P<0.05)。脑炎组的GAD-Ab与NMDA-Ab的表达阳性率为52.9%和38.8%,高于对照组的1.2%和2.4%(P<0.05)。在脑炎组中,Spearman相关分析显示神经发育评分与振幅整合脑电图背景活动分级、GAD-Ab与NMDA-Ab的表达阳性率都存在相关性(P<0.05)。结论:自身免疫性脑炎患儿可伴随有振幅整合脑电图异常,且神经发育滞后,两者具有显著相关性。 Objective:To investigate the correlation between brain electrical activity and behavioral neurodevelopment in patients with autoimmune encephalitis.Methods:From January 2015 to June 2020,85 cases of children with autoimmune encephalitis admitted to our hospital were collected as the encephalitis group,and 85 cases of healthy children who received physical examination in our hospital during the same period were selected as the control group.The background characteristics of(Amplitude Integrated Electroencephalogram,aEEG)in the two groups were recorded,and were to assess the behavioral neurodevelopment of children and given correlation analysis.Results:In the encephalitis group,there were 25 cases of CNV,31 cases of DNV,4 cases of BS,25 cases of CLV,and 0 case of FT;in the control group,there were 83 cases of CNV and 2 cases of DNV;the difference between two groups was statistically significant(P<0.05).The neurodevelopment score of the encephalitis group were significantly lower than that of the control group(P<0.05).The positive rates of GAD-Ab and NMDA-Ab in the encephalitis group were 52.9%and 38.8%,which were higher than those of the control group,1.2%and 2.4%(P<0.05).In the encephalitis group,Spearman correlation analysis showed that the neural development score were correlated the aEEG background activity grade,the positive rate of GAD-Ab and NMDA-Ab expression(P<0.05).Conclusion:Children with autoimmune encephalitis may be accompanied by abnormal aEEG and delayed neurodevelopment.There are significant correlation between the brain electrical activity and behavioral neurodevelopment.
