VPS与LPS治疗交通性脑积水的临床效果对比

Comparison of the Clinical Efficacy of Ventriculo-peritoneal Shunt and Lumbo-peritoneal Shunt in the Treatment of Communicating Hydrocephalus

目的:探讨脑室-腹腔分流术(VPS)与腰大池-腹腔分流术(LPS)治疗交通性脑积水的临床效果。方法:选取2018年2月-2020年2月收治的90例交通性脑积水患者作为研究对象,按照患者的治疗方案分为两组。其中VPS组45例,采用VPS治疗;LPS组45例,采用LPS治疗。比较两组治疗效果、NIHSS评分、术后并发症发生情况。结果:VPS组治疗总有效率80.00%,LPS组治疗总有效率82.22%,对比差异无统计学意义(P>0.05)。治疗前两组患者NIHSS评分对比差异无统计学意义(P>0.05);治疗后LPS组NIHSS评分低于VPS组,差异有统计学意义(P<0.05)。VPS组术后感染、堵管、意识障碍等总发生率为28.89%,高于LPS组的11.11%,差异有统计学意义(P<0.05)。结论:采用LPS治疗交通性脑积水的临床效果较好,相较于VPS治疗其并发症风险更低,患者预后更佳,值得推广。

Objective:To investigate the clinical effect of ventriculo-peritoneal shunt (VPS) and lumbar-peritoneal shunt (LPS) in the treatment of communicating hydrocephalus.Method:A total of 90 patients with traffic hydrocephalus from February 2018 to February 2020 were selected as the research objects,and they were divided into two groups according to the difference of treatment schemes.VPS group (n=45) was treated with VPS;LPS group (n=45) was treated with LPS.The curative effect,NIHSS scores and postoperative complications of the two groups were compared.Result:the total effective rate of VPS group and LPS group was 80.00% and 82.22%,respectively,with no significant difference (P>0.05).Before treatment,there was no significant difference in NIHSS score between the two groups (P>0.05);after treatment,the NIHSS score in LPS group was lower than that in VPS group,the difference was statistically significant (P<0.05).The total incidence of postoperative infection,tube blockage and disturbance of consciousness in VPS group was 28.89%,which was higher than 11.11% in LPS group (P<0.05).Conclusion:The clinical effect of LPS in the treatment of communicating hydrocephalus is better.Compared with VPS,the risk of complications is lower and the prognosis is better,which is worthy of promotion.
