自制透析管路固定夹在血液透析患者导管固定中的应用

Study on the Application of Self-made Fixing Clamp of Dialysis Tube in Hemodialysis Patients

目的:观察自制透析管路固定夹在血液透析患者导管固定中的应用效果,为临床治疗提供参考。方法:选取2019年6月-2020年6月在笔者所在医院接受血液透析治疗的60例患者作为研究对象,按随机数表法分为对照组和观察组,各30例。对照组采用传统胶布固定导管,观察组应用自制透析管路固定夹固定导管(专利号:ZL 2019 2 1991830.X),观察比较两组患者的医疗物品损坏率、穿刺口疼痛评分、导管穿刺点渗血率、导管相关感染率、导管移动距离及满意度。结果:观察组的气垫损坏率、床垫损坏率、床单损坏率、管道损坏率均为0,明显低于对照组的60.00%、50.00%、66.67%、13.33%,差异均有统计学意义(P<0.05);观察组的穿刺口疼痛评分、导管穿刺点渗血率、导管相关感染率、导管移动距离分别为(3.72±1.02)分、6.67%、3.33%、(0.39±0.17)cm,分别优于对照组的(5.17±1.78)分、30.00%、20.00%、(0.82±0.26)cm,差异均有统计学意义(P<0.05);观察组的总满意度为93.33%,明显高于对照组的73.33%,差异有统计学意义(P<0.05)。结论:自制透析管路固定夹在血液透析患者导管固定中效果显著,可明显降低医疗物品损坏率及导管相关感染率,缓解疼痛,增加导管稳定性,在临床中值得推广应用。

Objective:To analyze the application of self-made fixing clip of dialysis tube in hemodialysis patients,so as to provide basis for clinical treatment.Method:A total of 60 patients with hemodialysis in our hospital were selected as the research objects from June 2019 to June 2020.According to random number table method,they were divided into the control group and the observation group,30 cases in each group.The control group was fixed with traditional adhesive tape,while the observation group was fixed with self-made dialysis pipeline clamp (Patent No.:ZL 2019 2 1991830.X).The difference of damage rate of medical articles,pain score of puncture site,blood leakage rate of catheter puncture point,catheter-related infection rate,catheter movement damage and satisfaction distance between the two groups were observed.Result:The air cushion damage rate,mattress damage rate,sheet damage rate and pipeline distance rate of the observation group were 0,which were significantly lower than 60.00%,50.00%,66.67% and 13.33% of the control group,the differences were statistically significant (P<0.05);the pain score of puncture site,bleeding rate of catheter puncture point,catheter-related infection rate and catheter movement distance of observation group were (3.72±1.02) points,6.67%,3.33% and (0.39±0.17) cm respectively,which were significantly better than (5.17±1.78) points,30.00%,20.00% and (0.82±0.26) cm of the control group,with statistically significant differences (P<0.05);the satisfaction rate of the observation group was 93.33%,which was significantly higher than 73.33% of the control group,the difference was statistically significant (P<0.05).Conclusion:The effect of self-made fixation clip of dialysis pipeline in hemodialysis patients is significant,which can significantly reduce the damage rate of medical supplies and catheter-related infection rate,relieve pain and increase the stability of catheter,which is worthy of promotion and application in clinic.
