基于“脾以运为健,胃以通为补”理论运用健脾消疳汤治疗小儿疳证的效果探讨

Effects of Jianpi Xiaogan Decoction in the Treatment of Infantile Malnutrition Syndrome Based on the Theory of“Activating Spleen for Health and Harmonizing Stomach for Replenishment”

目的:探讨基于"脾以运为健,胃以通为补"理论运用健脾消疳汤治疗小儿疳证的效果。方法:选用2017年11月-2019年11月经本院收治的112例疳证患儿,行随机数字表法分为对照组及观察组,对照组给予西医常规治疗,观察组在对照组基础上,基于"脾以运为健,胃以通为补"理论,准确辨证分型,合理取方施治,定于健脾消疳汤用以治疗,持续治疗4周,比较两组疗效、症候积分、血清及微量元素指标及不良反应。结果:观察组总有效率达94.64%,高于对照组的80.36%(P<0.05);经治疗,两组症候中面色、腹胀、体重及食欲积分均有下降,观察组面色、腹胀及食欲积分均低于对照组(P<0.05),体重差异无统计学意义(P>0.05);两组HGB、RBC、Ca和Zn指标均显上升,观察组较对照组更高(P<0.05);观察组不良反应总发生率为1.79%,相似于对照组的5.38%(P>0.05)。结论:基于"脾以运为健,胃以通为补"理论运用健脾消疳汤治疗小儿疳证,效果确切,且气血改善,促进微量元素吸收,不良反应降低。

Objective:To explore the effects of Jianpi Xiaogan Decoction in the treatment of infantile malnutrition syndrome based on the theory of“activating spleen for health and harmonizing stomach for replenishment”.Method:Totally 112 children patients with malnutrition syndrome who were admitted to this hospital from November 2017 to November 2019 were selected and divided into the control group and the observation group according to the random number table method.The control group was given conventional western medicine treatment while the observation group was given Jianpi Xiaogan Decoction through accurate dialectical classification and rational use of prescriptions for treatment based on the theory of“activating spleen for health and harmonizing stomach for replenishment”,and the two groups were continuously treated for 4 weeks.The efficacy,symptoms scores,serum and trace element indicators and adverse reactions were compared between the two groups.Result:The total effective rate of the observation group was up to 94.64%,which was higher than 80.36%of the control group(P<0.05).After treatment,the scores of symptoms of complexion,abdominal distension,weight and appetite of the two groups were decreased,and the scores of complexion,abdominal distension and appetite of the observation group were lower than those of the control group(P<0.05),but there was no significant difference in the body weight(P>0.05).The HGB,RBC,Ca and Zn were increased significantly in the two groups,and the indexes of the observation group were higher than those of the control group(P<0.05).The total incidence rate of adverse reactions was 1.79%in the observation group,which was similar to 5.38%in the control group(P>0.05).Conclusion:Based on the theory of“activating spleen for health and harmonizing stomach for replenishment”,Jianpi Xiaogan Decoction has a definite efficacy in the treatment of infantile malnutrition syndrome,and it can improve Qi and blood,promote absorption of trace elements,and reduce adverse reactions.
