摘要
目的探讨A型肉毒毒素联合普瑞巴林治疗带状疱疹后遗神经痛的临床效果。方法选择2018年2月至2019年12月于我院皮肤科门诊确诊为带状疱疹后遗神经痛的44例患者作为研究对象,采用随机数字表法将其分为参照组和观察组,各22例。参照组给予普瑞巴林治疗,观察组在对照组基础上给予A型肉毒毒素治疗。比较两组的疼痛程度、炎症因子水平、临床疗效及不良反应发生情况。结果治疗2、4、8、12周后,观察组的VAS评分低于参照组,差异具有统计学意义(P<0.05)。治疗12周后,观察组的IL-1β、IL-6、TNF-α水平低于参照组,IL-10水平高于参照组,差异具有统计学意义(P<0.05)。观察组的治疗总有效率为95.45%,高于参照组的63.64%,差异具有统计学意义(P<0.05)。两组的不良反应总发生率比较,差异无统计学意义(P>0.05)。结论A型肉毒毒素联合普瑞巴林治疗带状疱疹后遗神经痛的效果显著,可缓解患者的疼痛程度,减轻炎症反应,且不良反应较少。
Objective To investigate the clinical effect of botulinum toxin A combined with pregabalin in the treatment of postherpetic neuralgia.Methods From February 2018 to December 2019,44 patients with postherpetic neuralgia diagnosed in the outpatient department of dermatology in our hospital were selected as the research objects,and the patients were divided into reference group and observation group by random number table method,with 22 cases in each group.The reference group was given pregabalin treatment,and the observation group was given botulinum toxin A treatment on the basis of the reference group.The degree of pain,levels of inflammatory factors,clinical efficacy and adverse reactions were compared between the two groups.Results After 2,4,8 and 12 weeks of treatment,the VAS scores in the observation group were lower than those in the reference group,and the differences were statistically significant(P<0.05).After 12 weeks of treatment,the levels of IL-1β,IL-6,TNF-αin the observation group were lower than those in the reference group,the level of IL-10 was higher than that in the reference group(P<0.05).The total effective rate of treatment in the observation group was 95.45%,which was higher than 63.64%in the reference group,the difference was statistically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Botulinum toxin A combined with pregabalin in the treatment of postherpetic neuralgia has a significant effect,which can relieve the degree of pain and inflammatory reaction,and with less adverse reactions.
