定量动态增强磁共振成像对良、恶性骨肿瘤的鉴别诊断价值 被引量:4

Value of quantitative dynamic contrast-enhanced MRI in differential diagnosis of benign and malignant bone tumors

摘要 目的探讨定量动态增强磁共振成像(DCE-MRI)对良、恶性骨肿瘤的鉴别诊断价值。方法选取2018年2月至2020年3月收治的50例骨肿瘤患者作为研究对象,均行定量DCE-MRI及病理检查。以病理检查为金标准,分析定量DCE-MRI对骨肿瘤患者的诊断结果。根据病理检查结果将骨肿瘤患者分为良性肿瘤组和恶性肿瘤组,比较两组的定量DCE-MRI参数。结果50例骨肿瘤患者中,32例经病理检查诊断为恶性肿瘤,18例诊断为良性肿瘤;30例经定量DCE-MRI诊断为恶性肿瘤,20例诊断为良性肿瘤。定量DCE-MRI诊断骨肿瘤的灵敏度为87.50%,特异度为88.89%,准确度为88.00%。良性肿瘤组的容量转运常数(Ktrans)、速率常数(Kep)低于恶性肿瘤组,差异具有统计学意义(P<0.05);两组的血管外细胞外间隙容积分数(Ve)比较,差异无统计学意义(P>0.05)。结论定量DCE-MRI可较好地鉴别诊断骨肿瘤的良、恶性,为患者接受针对性的治疗提供参考依据,值得在临床推广和应用。 Objective To investigate the value of quantitative dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI)in differential diagnosis of benign and malignant bone tumors.Methods From February 2018 to March 2020,50 patients with bone tumors were selected as the research objects,all of them underwent quantitative DCE-MRI and pathological examination.The results of quantitative DCE-MRI in the diagnosis of bone tumors were analyzed with pathological examination as the gold standard.According to the pathological esamination results,patients with bone tumors were divided into benign tumor group and malignant tumor group,and the quantitative DCE-MRI parameters of the two groups were compared.Results Among 50 patients with bone tumors,32 cases were diagnosed as malignant tumors by pathological examination,and 18 cases were diagnosed as benign tumors;30 cases were diagnosed as malignant tumor by quantitative DCE-MRI,and 20 cases were diagnosed as benign tumor.The sensitivity,specificity and accuracy of quantitative DCE-MRI in diagnosis of bone tumors were 87.50%,88.89%and 88.00%,respectively.The volume transfer constan(Ktrans)and the rate constants(Kep)of the benign tumor group were lower than those of the malignant tumor group,the differences were statistically significant(P<0.05);there was no significant difference in the extravascular extracellular volume fraction(Ve)between the two groups(P>0.05).Conclusion Quantitative DCE-MRI can better differentiate benign and malignant bone tumors,and provide reference for patients to receive targeted treatment,which is worthy of clinical promotion and application.
