亚临床甲状腺功能减退症合并妊娠期糖尿病对患者生化指标及妊娠结局的影响

Influences of Subclinical Hypothyroidism with Gestational Diabetes Mellitus on Biochemical Parameters and Pregnancy Outcome

目的研究亚临床甲状腺功能减退症合并妊娠期糖尿病对孕妇生化指标和妊娠结局的影响。方法以2017年4月至2019年3月本院妇产科接诊的亚临床甲状腺功能减退症孕妇50例为对照组,并以本院妇产科同期接诊的亚临床甲状腺功能减退症合并妊娠期糖尿病50例为试验组。对两组的生化指标进行检测,同时对其检测结果和妊娠结局作出比较。结果试验组的TG、TC和LDL-C水平比对照组高,P<0.05。试验组的HDL-C水平同对照组比较无显著差异,P>0.05。试验组的血清促甲状腺激素水平为(10.62±3.57)mIU/L,比对照组的(6.09±1.73)mIU/L高,P<0.05。试验组的超敏C反应蛋白、肿瘤坏死因子α以及白细胞介素-6水平均比对照组高,P<0.05。试验组的不良妊娠结局发生率比对照组高,P<0.05。结论亚临床甲状腺功能减退症合并妊娠期糖尿病孕妇存在严重的脂质代谢异常的情况,且容易出现妊娠结局预后不良的情况,故对于该类孕妇,需加强对其甲状腺功能和脂质代谢进行监测的力度,同时予以个体化的干预,减少不良妊娠结局发生的风险。

Objective To study the effects of subclinical hypothyroidism combined with gestational diabetes mellitus on biochemical indexes and pregnancy outcomes of pregnant women.Method From April 2017 to March 2019,50 pregnant women with subclinical hypothyroidism received by the department of gynecology and obstetrics in our hospital were taken as the control group,and 50 pregnant women with subclinical hypothyroidism and gestational diabetes mellitus received by the department of gynecology and obstetrics in our hospital at the same time were taken as the experimental group.The biochemical indexes of the two groups were tested,and the results of the test and pregnancy outcomes were compared.Result The levels of TG,TC and LDL-C in the experimental group were higher than those in the control group,P<0.05.The HDL-C level of the experimental group was not significantly different from that of the control group,P>0.05.The serum thyroid-stimulating hormone level in the experimental group was(10.62±3.57)mIU/L,which was higher than the control group(6.09±1.73)mIU/L,P<0.05.The levels of hypersensitivity C-reactive protein,tumor necrosis factor alpha and interleukin 6 in the experimental group were higher than those in the control group,P<0.05.The incidence of adverse pregnancy outcomes in the experimental group was higher than that in the control group,P<0.05.Conclusion For pregnant women with subclinical hypothyroidism and gestational diabetes mellitus,there are serious abnormal lipid metabolism,and they are prone to poor prognosis of pregnancy outcomes.Therefore,it is necessary to strengthen the monitoring of thyroid function and lipid metabolism of these pregnant women,and to give individuals at the same time.Intervention can reduce the risk of adverse pregnancy outcomes.
