护理专案对结直肠癌患者口服化疗药物的服药依从性及合理用药自我效能的影响

Effect of team nursing on drug compliance and self-efficacy of rational drug use in colorectal cancer patients receiving oral chemotherapy

目的探讨护理专案对结直肠癌患者口服化疗药物的服药依从性及合理用药自我效能的影响。方法纳入2018年5月至2020年4月于我院消化外科接受结直肠癌口服化疗药物治疗的84例患者作为研究对象,按照入院时间分为两组,以2018年5月至2019年4月入选的42例为对照组(接受常规护理),以2019年5月至2020年4月入选的42例为观察组(接受护理专案护理),两组均持续干预至第8个化疗周期结束。比较两组服药依从性情况(Morisky服药依从性量表得分、服药依从性优良率)及合理用药自我效能量表得分。结果干预前,两组Morisky服药依从性量表得分、服药依从性优良率比较差异均无统计学意义(均P>0.05);干预后,两组Morisky服药依从性量表得分均高于组内干预前,观察组Morisky服药依从性量表得分、服药依从性优良率均高于对照组(均P<0.05)。干预前,两组困难情况下、不确定情况下的合理用药自我效能得分及总分比较,差异均无统计学意义(均P>0.05);干预后,两组前述指标得分均高于组内干预前,观察组前述得分均高于对照组(均P<0.05)。结论护理专案应用于结直肠癌患者口服化疗药物的用药护理可有效提高患者服药依从性,增强合理用药自我效能。

Objectives To investigate the effect of team nursing on drug compliance and self-efficacy of rational drug use in colorectal cancer patients receiving oral chemotherapy.Methods Eighty-four patients with colorectal cancer receiving oral chemotherapy at the Department of Gastroenterology in our hospital between May 2018 and April 2020 were recruited and assigned to two groups according to the time of admission.Forty-two patients admitted between May 2018 and April 2019 were assigned to the control group and received routine nursing,and 42 patients admitted between May 2019 and April 2020 were assigned to the treatment group and received team nursing.For both groups,the intervention was given until the end of the eighth cycle of chemotherapy.Drug compliance was evaluated using the Morisky Medication Adherence Scale(MMAS),and high and medium adherence rates.Rational drug use was evaluated using the Self-efficacy for Appropriate Medication Use Scale(SEAMS).Results The two groups did not differ in MMAS total score and the rate of high and medium adherence before intervention(P>0.05).After the intervention,MMAS total score increased significantly within both groups,and the treatment group had a significantly higher MMAS total score and higher rate of high and medium adherence(P<0.05).Before the intervention,the two groups were similar in the total score of the SEAMS and sub-scores of SEAMS in difficult situations and uncertain situations(P>0.05).After the intervention,these scores increased significantly within both groups and were significantly higher in the treatment group than in the control group(P<0.05).Conclusion Team nursing could improve patients’drug compliance and self-efficacy of rational drug use in colorectal cancer patients receiving oral chemotherapy.
