防己黄芪汤治疗原发性肾病综合征临床观察

Clinical Observation on Modified Fangji Huangqi Decoction in the Treatment of Primary Nephrotic Syndrome

目的探讨防己黄芪汤加味治疗原发性肾病综合征的临床效果。方法选取2018年6月—2019年5月辽宁省锦州市中医医院接收治疗的82例原发性肾病综合征患者作为本次研究对象,对患者编号,共1~82号,其中奇数号作为对照组,采用常规西药治疗;偶数号作为观察组,在西药治疗基础上增加防己黄芪汤加味,每组41例。比较2组临床治疗效果,治疗前后相关生化指标、血清肌酐(Serum creatinine,SCr)、内生肌酐清除率(Endogenous creatinine clearance rate,CCr)、血尿素氮(Blood urea nitrogen,BUN)、凝血酶原时间(Prothrombin time,PT)及血小板计数。结果观察组的治疗总有效率为92.68%(38/41),高于对照组的78.05%(32/41),差异有统计学意义(P<0.05);观察组治疗后的尿蛋白定量、总胆固醇、SCr、BUN水平低于对照组,血浆白蛋白、CCr水平高于对照组,PT长于对照组,差异均有统计学意义(P<0.05)。结论对原发性肾病综合征患者实施防己黄芪汤加味治疗,能提高患者的治疗效果,改善肾功能生化指标水平,值得临床推广应用。

Objective To investigate the clinical effect of Fangji Huangqi decoction in treating primary nephrotic syndrome.Methods From June 2018 to May 2019,82 patients with primary nephrotic syndrome who were treated in our hospital were selected as the research objects.The patients were numbered 1-82,and the odd numbers was the control group of 41 cases,and were treated with conventional western medicine.The even number was the observation group.41 cases were treated with western medicine and Fangji Huangqi decoction.The clinical effect,relevant biochemical indexes,serum creatinine(SCR),endogenous creatinine clearance rate(CCR),blood urea nitrogen(BUN),prothrombin time(PT)and platelet count between the two groups was compared.Results The total effective rate of the observation group was 92.68%(38/41),which was higher than that of the control group(78.05%,32/41),and the difference was statistically significant(P<0.05).After treatment,the levels of urinary protein,total cholesterol,SCR and BUN in the observation group were lower than those in the control group,the levels of plasma albumin and CCR were higher than those in the control group,the PT was longer than that in the control group,and the difference was statistical significance(P<0.05).Conclusion Fangji Huangqi decoction in treating primary nephrotic syndrome can improve the therapeutic effect of patients,improve the level of renal function biochemical indicators,and is worthy of clinical application.
