济南地区RhD阴性血临床供应状况分析

Analysis of clinical supply of RhD negative blood in Jinan area

目的统计分析济南地区RhD阴性血液临床发放情况,为建立Rh阴性血合理、有效的管理办法提供参考,以便更好地保障RhD阴性患者输血需求。方法收集山东省血液中心2010-2018年临床供应红细胞制品和阴性红细胞数据,统计分析济南地区RhD阴性血液用量特点。结果本中心供应临床红细胞数基本上呈递增形式,2010-2018年红细胞供应量增长率为32.5%;各年度供应济南地区的RhD阴性血发放总量由2010年的975U上升至2018年的1208U,增长率为23.9%,A、B、O、AB型RhD阴性血液供应量比例约为3∶3∶3∶1。A、B、O、AB各型RhD阴性红细胞占比分别为0.67%、0.62%、0.70%和0.77%,以AB型RhD阴性占比最高。RhD阴性红细胞供应量以普通红细胞为主,2010-2018年冰冻红细胞发放占比5.4%~43.7%。结论2010-2018年山东省济南地区RhD阴性血临床用量增加,比例高于RhD人群分布比例,需进一步加强RhD阴性血液管理,更好地保障RhD阴性患者输血需求。

Objective This study aimed to statistically analyze the clinical distribution of RhD-negative blood in Jinan,and to provide a reference for establishing a reasonable and effective management method for Rh-negative blood,so as to better guarantee the transfusion needs of RhD-negative patients.Methods From 2010 to 2018,clinical supply of red blood cell products and negative red blood cell data of the center were collected,and the characteristics of RhD negative blood consumption in Jinan were statistically analyzed.Results The number of clinical red blood cells supplied by the center was basically increasing,with the growth rate of 32.5%from 2010 to 2018.The total amount of RhD-negative blood supply in Jinan increased from 975 Uin 2010 to 1208 Uin 2018,the growth rate was 23.9%,and the ratio of A、B、O、AB types was about 3∶3∶3∶1.The proportions of RhD-negative red blood cells of A,B,O and AB were 0.67%,0.62%,0.70%and 0.77%,and AB type RhD-negative accounted for the highest proportion.The supply of RhD-negative red blood cells was mainly composed of ordinary red blood cells,and the distribution of frozen red blood cells accounted for5.4%to 43.7%from 2010 to 2018.Conclusions In recent years,the clinical dosage of RhD-negative blood in Jinan has increased,and the proportion is higher than the distribution ratio of RhD population.It is necessary to further strengthen the management of RhD-negative blood and better guarantee the transfusion needs of RhD-negative patients.
