广东省寮步镇老年高血压人群臂间血压差异的相关因素研究

Study on the Associated Factors of the Difference in Inter-arm Blood Pressure of Elderly Hypertensive Population in Liaobu Town,Guangdong Province

目的探讨老年高血压人群臂间血压差异增大的相关因素。方法本研究为横断面研究,对广东省东莞市寮步镇参与2013年1月1日-12月31日年度体检的4744例老年原发性高血压患者进行调查,测量双臂肱动脉血压并计算臂间血压差异。通过单因素线性回归和多因素线性回归统计分析影响臂间血压差异的相关因素。结果单因素线性回归分析显示,收缩压和舒张压为影响臂间血压差异的相关因素(P<0.001)。多因素线性回归分析显示,随着收缩压升高(β=0.069,P<0.001)或舒张压下降(β=-0.030,P<0.001),臂间收缩压差异升高,随着年龄增大(β=0.031,P<0.001)、收缩压下降(β=-0.020,P<0.001)、舒张压升高(β=0.119,P<0.001)、心率减慢(β=-0.015,P=0.001)、总胆固醇下降(β=4.259,P=0.009)、高密度脂蛋白胆固醇升高(β=0.533,P=0.005)、吸烟(β=0.321,P>0.043),臂间舒张压差异升高。结论影响老年高血压人群臂间血压差异增大的相关因素为收缩压、舒张压、心率、血脂水平和吸烟。

Objective To explore the associated factors of the increased inter-ann blood pressure difference(IAD)in elderly hypertensive population.Methods This study was a cross-sectional study.We measured the bilateral brachial blood pressure and calculated the IAD in 4,744 elderly essential hypertensive patients,who were recruited during January 1st to December 31st of 2013.The univariate and multivariate linear regression models was adopted to evaluatethe associated factors of IAD.Results Univariate linear regression models indicated that systolic blood pressure(SBP)and diastolic blood pressure(DBP)are the associated factors of IAD(P<0.001).Multivariate linear regression models showed that increased SBP(β=0.069,P<0.001)and decreased DBP(β=-0.030,P<0.001)are associated with increased systolic IAD;aging(β=0.031,P<0.001),decreased SBP(β=-0.020,P<0.001),increased DBP(β=0.119,P<0.001),decreased heart rate(β=-0.015,P=0.001),decreased total cholesterol(β=0.259,P=0.009),increased high density lipoprotein cholesterol(β=0.533,P=0.005)and smoking(β=0.321,P=0.043)are associated with increased diastolic IAD.Conclusion In elderly hypertensive population,the associated factors of the increased IAD include SBP,DBP,heart rate,serum lipid levels and smoking.
