摘要
目的探讨冠心病患者醛固酮(aldosterone,ALD)、成纤维细胞生长因子21(recombinant fibroblast growth factor 21,FGF21)及肾素活性(plasma renin activity,PRA)水平变化及对发生心血管事件的预测价值。方法以2017年12月-2018年12月我住院部治疗的102例冠心病患者为研究对象,分为发生心血管事件组52例、未发生心血管事件组50例。对比2组ALD、FGF21、PRA水平,探讨影响冠心病患者发生心血管事件的相关危险因素;绘制ROC曲线,分析ALD、FGF21、PRA及三者联合检测对冠心病患者发生心血管事件的预测价值。结果发生心血管事件组ALD、FGF21及PRA水平均显著高于未发生心血管事件组,比较差异具有统计学意义(P<0.05)。高血压史、糖尿病史、吸烟、酗酒、ALD(异常升高)、FGF21(异常升高)、PRA(异常升高)为影响冠心病患者发生心血管事件的相关危险因素,比较差异具有统计学意义(P<0.05)。ALD、FGF21、PRA及三者联合检测ROC曲线下面积分别为:0.710、0.880、0.919、0.955,联合检测ROC曲线下面积值最佳。结论ALD、FGF21、PRA在发生心血管事件及未发生心血管事件的冠心病患者中的表达存在显著差异,三者联合检测对预测冠心病患者发生心血管事件具有较高的敏感性。
Objective To explore the changes in the levels of aldosterone(ALD),recombinant fibroblast growth factor 21(FGF21)and plasma renin activity(PRA)in patients with coronary heart disease,and their predictive value for cardiovascular events.Method The 102 patients with coronary heart disease who were treated in my inpatient department from December 2017 to December 2018 were selected as the research objects,they were divided into two groups:52 cases with cardiovascular events and 50 cases with non-cardiovascular event group.The levels of ALD,FGF21,and PRA were compared between the two groups to explore the related risk factors affecting cardiovascular events in patients with coronary heart disease.The ROC curve was drawn to analyze the predictive value of ALD,FGF21,PRA and the combined detection of them for the occurrence of cardiovascular events in patients with coronary heart disease.Result The levels of ALD,FGF21 and PRA in the cardiovascular event group were significantly higher than those in the non-cardiovascular event group,and the difference was statistically significant(P<0.05).History of hypertension,history of diabetes,smoking,alcoholism,ALD(abnormal increase),FGF21(abnormal increase),PRA(abnormal increase)were related risk factors affecting cardiovascular events in patients with coronary heart disease,and the differences were statistically significant(P<0.05).The area under the ROC curve of ALD,FGF21,PRA and the combined detection of them were 0.710,0.880,0.919,0.955,and the area under the ROC curve of the combined detection was the best.Conclusion There are significant differences in the expression of ALD,FGF21,and PRA in coronary heart disease patients with cardiovascular events and patients without cardiovascular events.The combined detection of them has high sensitivity for predicting cardiovascular events in patients with coronary heart disease.
