虚拟情景模拟训练结合中频脉冲电治疗对偏瘫患者上肢功能的影响 被引量:8

Effect of virtual scene simulation training combined with midium frequency impulse electrotherapy on upper limb function of hemiplegic patients

摘要 目的探讨虚拟情景模拟训练结合中频脉冲电治疗对偏瘫患者上肢功能和日常生活能力的影响。方法 2019年3月-10月纳入偏瘫患者50例,按随机数表法分为试验组和对照组,每组各25例。对照组采用常规康复训练,试验组在常规康复训练的基础上增加虚拟情景模拟训练和中频脉冲电治疗。两组分别于治疗前和治疗8周后采用Fugl-Meyer上肢运动功能评分(Fugl-Meyer Assessment-Upper Extremities,FMA-UE)、简易上肢功能检查量表(Simple Test for Evaluating Hand Function,STEF)和改良Barthel指数(Modified Barthel Index,MBI)评定患者的上肢运动功能和日常生活能力。结果试验组和对照组患者治疗前FMA-UE评分分别为(22.88±5.18)、(23.44±6.26)分,STEF评分分别为(40.12±4.82)、(41.44±4.54)分,MBI评分分别为(51.40±7.29)、(48.60±7.00)分,各项评分组间差异均无统计学意义(P>0.05);治疗8周后,FMA-UE评分分别为(39.48±6.35)、(33.52±6.53)分,STEF评分分别为(59.08±7.54)、(52.52±5.83)分,MBI评分分别为(71.00±8.78)、(62.40±9.37)分,各项评分组间差异均有统计学意义(P<0.05)。治疗8周后,两组患者的FMA-UE、STEF和MBI评分较治疗前均有改善(P<0.05),且试验组各项评分改善情况优于对照组(P<0.05)。两组患者治疗后均未出现脑卒中复发、皮肤电灼伤等不良反应。结论虚拟情景模拟训练结合中频脉冲电治疗能有效提高偏瘫患者的上肢运动功能,改善生活质量。 Objective To investigate the effect of virtual scene simulation training combined with midium frequency impulse electrotherapy on upper limb function and daily living ability of hemiplegia patients. Methods From March to October 2019, 50 hemiplegic patients were recruited and randomly assigned to the trial group and the control group, with 25 patients in each group. The control group was given routine rehabilitation training, while the trial group was given virtual scene simulation training and medium frequency impulse electrotherapy on the basis of routine rehabilitation training. The Fugl-Meyer Assessment-Upper Extremities(FMA-UE), Simple Test for Evaluating Hand Function(STEF), and Modified Barthel Index(MBI) were used to assess patients’ upper limb function and daily living ability before treatment and after 8 weeks of treatment. Results Before treatment, the FMA-UE, STEF, and MBI scores of the trial group vs. the control group were 22.88±5.18 vs. 23.44±6.26, 40.12±4.82 vs. 41.44±4.54, and 51.40±7.29 vs.48.60±7.00, respectively, and none of the between-group differences was statistically significant(P>0.05);after 8 weeks of treatment, the FMA-UE, STEF, and MBI scores of the two groups were 39.48±6.35 vs. 33.52±6.53, 59.08±7.54 vs.52.52±5.83, and 71.00±8.78 vs. 62.40±9.37, respectively, and all of the between-group differences were statistically significant(P<0.05). After 8 weeks of treatment, the FMA-UE, STEF and MBI scores of the two groups of patients were significantly improved compared with those before treatment(P<0.05), and the improvement of each score of the trial group was significantlybetter than that of the control group(P<0.05). No stroke recurrence, electric burn, or other adverse reactions occurred in the two groups after treatment. Conclusion Virtual scene simulation training combined with midium frequency impulse electrotherapy can effectively improve the upper limb function of patients with hemiplegia and improve their quality of life.
