大脑前动脉A1段管径及分支血管角度与前交通动脉瘤关系的研究

摘要 目的探索大脑前动脉(ACA)A1段管径、分支血管角度与前交通动脉瘤(ACoA)的相关性。方法选择2013年1月至2019年8月浙江省立同德医院经数字减影血管成像或CT血管成像(CTA)证实为ACA A1段优势供血的ACoA患者(ACoA组)和无ACoA的求诊者(对照组),各42例,分析两组受检者的头颅CTA图像,测量ACoA颈径、最大横径、最大垂直高度及纵横径比值。分析ACA A1段优势供血侧ACA A1段管径、分支血管角度,并分析其与ACoA的相关性。结果ACoA组ACA A1段管径、分支血管角度均大于对照组,差异均有统计学意义(均P<0.05)。ACA A1段管径与ACoA纵横径比值呈负相关(r=-0.508,P<0.05)。ACA A1段管径、分支血管角度与ACoA颈径、最大横径、最大垂直高度,ACA A1段分支血管角度与ACoA纵横径比值均不相关(均P>0.05)。ACoA组中ACA A1段管径与分支血管角度呈正相关(r=0.345,P<0.05)。结论ACA A1段管径、分支血管角度与ACoA形成相关。 Objective To investigate the relationship of diameter and bifurcation angle of A1 segment of anterior cerebral artery(ACA)with anterior communicating aneurysms(ACoA).Methods Images of cerebral computerized tomography angiography(CTA)for 42 patients with ACoA along with dominance blood supply of ACA A1 and 42 controls were reviewed from January 2013 to August 2019 in Tongde Hospital of Zhejiang Province.The radiological parameters included diameter and bifurcation angle of ACA A1,the maximum vertical height,maximum transverse width,neck width of ACoA and ratio of the height to neck width of ACoA were measured.The differences in diameter and bifurcation angle of ACA A1 between ACoA and control group and the relationship of the diameter and bifurcation angle of ACA A1 to the shape of ACoA were analyzed.Results There were significant differences in the diameter and bifurcation angle of ACA A1 between ACoA and control group(P<0.05).Diameter of ACA A1 was negatively correlated with ratio of the height to neck width(r=-0.508,P<0.05).Diameter and bifurcation angle of ACA A1 were not correlated with the neck diameter,maximum transverse diameter,and maximum vertical height of ACoA(P>0.05).Moreover,bifurcation angle of ACA A1 was not correlated with the ratio of the height to neck width(P>0.05).The diameter of ACA A1 was positively correlated with bifurcation angle of ACA A1(r=0.345,P<0.05).Conclusion Diameter and bifurcation angle of ACA A1 may be associated with the formation of ACoA.
