能谱CT最佳单能量成像结合低碘含量对比剂扫描方案在肝细胞癌诊断中的可行性和应用价值

Feasibility and Application Value of Optimal Single-energy Imaging of Energy Spectrum CT Combined with Low Iodine Content ContrastAgent Scanning Scheme in the Diagnosis of Hepatocellular Carcinoma

目的:探讨能谱CT最佳单能量成像结合低碘含量对比剂扫描方案在肝细胞癌诊断中的可行性和应用价值。方法:选取2018年4月~2019年10月某院肝细胞癌患者60例,将患者分为对照组和观察组各30例,对照组行能谱CT最佳单能量成像,观察组在对照组的基础上结合低碘含量对比剂,对比两组患者的图像对比度、图像噪声、对比噪声对比值、动脉期评分和门静脉期评分。结果:观察组图像的对比度、图像噪声、对比噪声对比值均高于对照组,差异具有显著性(P<0.05);两组患者主观评价指标包括动脉期评分、门脉期评分,观察组均优于对照组(P<0.05)。结论:能谱CT单能量成像结合碘对比剂用量个体化方案可以保证良好的肝细胞癌图像质量,对比度以及图像噪声较高,动脉期、门脉期评分优良,在临床术前诊断中应用前景广泛。

Objective:To investigate the feasibility and application value of the optimal single-energy imaging combined with low iodine content contrast agent scanning scheme in the diagnosis of hepatocellular carcinoma.Methods:60 patients with hepatocellular carcinoma in a hospital from April 2018 to October 2019 were selected and divided into control group and observation group according to random number table method,with 30 cases in each group.The control group underwent the optimal single-energy imaging of energy spectrum CT,while the observation group combined with low iodine content contrast agent on the basis of the control group,then the image contrast,image noise,contrast noise contrast value,arterial phase score and portal venous phase score between the two groups were compared.Results:The contrast of image,image noise and contrast noise of the observation group were higher than those of the control group,and the difference was significant(P<0.05).The subjective evaluation indexes contrast including arterial phase score and portal phase score of the observation group were better than those of the control group(P<0.05).Conclusion:The energy spectrum CT single-energy imaging combined with the individualized scheme of iodine contrast agent dosage can ensure good image quality,high contrast and image noise of hepatocellular carcinoma,and excellent scores of arterial and portal phases have broad application prospects in the clinical preoperative diagnosis.
