紫杉醇单药及与小剂量阿帕替尼联合治疗一线化疗失败晚期胃癌的近期疗效比较

Comparison of short-term efficacy of low-dose apatinib combined with paclitaxel versus paclitaxel monotherapy on advanced gastric cancer with first-line chemotherapy failure

目的观察紫杉醇单药及与小剂量阿帕替尼联合治疗一线化疗失败晚期胃癌的近期疗效。方法将2018年1月至2019年12月平煤神马医疗集团总医院收治的92例一线化疗失败的晚期胃癌患者纳入研究,按照随机数表法均分为联合组(小剂量250 mg/d阿帕替尼联合紫杉醇治疗)和对照组(紫杉醇单药治疗),每组46例。比较两组患者临床疗效、无进展生存期(PFS)及不良反应发生情况。结果联合组疾病控制率(DCR)、客观缓解率(ORR)高于对照组(P<0.05)。联合组PFS水平高于对照组(P<0.05)。两组恶心呕吐、腹泻、高血压、血小板减少、白细胞减少、肝功能异常发生率比较,差异未见统计学意义(P>0.05)。结论小剂量阿帕替尼联合紫杉醇相较紫杉醇单药治疗能够提高一线化疗失败的晚期胃癌患者近期疗效,延长PFS,安全性良好。

Objective To compare the short-term efficacy of low-dose apatinib combined with paclitaxel and paclitaxel monotherapy on advanced gastric cancer with first-line chemotherapy failure.Methods A total of 92 advanced gastric cancer patients with first-line chemotherapy failure in Pingmei Shenma Medical Group General Hospital from January 2018 to December 2019 were included in the study.According to the random number table method,the patients were divided into combined group(low-dose 250 mg/d apatinib combined with paclitaxel)and control group(paclitaxel monotherapy),with 46 cases in each group.The clinical efficacy,progression free survival(PFS)and adverse reactions between the two groups were compared.Results The disease control rate(DCR)and objective remission rate(ORR)in combined group were higher than those in control group(P<0.05).The PFS level in combined group was higher than that in control group(P<0.05).There were no significant differences in the incidence rates of nausea and vomiting,diarrhea,hypertension,thrombocytopenia,leukopenia and abnormal liver function between the two groups(P>0.05).Conclusions Compared with paclitaxel alone,low-dose apatinib combined with paclitaxel can improve the short-term efficacy of advanced gastric cancer with first-line chemotherapy failure,prolong PFS and have good safety.
