血虚寒凝型糖尿病周围神经病变患者治疗中运用当归四逆汤加减治疗的临床价值

Clinical value of addition and subtraction of Danggui Sini decoction in the treatment of diabetic peripheral neuropathy with blood deficiency and cold coagulation

目的研究血虚寒凝型糖尿病周围神经病变(DPN)患者治疗中运用当归四逆汤加减治疗的临床价值。方法选取2018年6月至2019年11月本院收治的64例血虚寒凝型DPN患者作为研究对象,根据电脑随机盲选法分为观察组和对照组,每组32例。对照组给予常规西药治疗(甲钴胺胶囊、维生素B1、叶酸片)。观察组在对照组基础上应用当归四逆汤加减治疗。比较两组治疗前后中医症候积分、多伦多临床评分系统(TCSS)评分改善情况及正中神经、腓总神经传导速度。结果治疗后,两组中医症候积分与TCSS评分均低于治疗前,且观察组低于对照组(P<0.05);两组正中神经及腓总神经SNCV、MNCV均明显高于治疗前,且观察组高于对照组(P<0.05)。结论运用当归四逆汤加减治疗血虚寒凝型糖尿病周围神经病变疗效显著,可有效改善患者临床症状,提升患者神经传导速度,应用价值较高,值得临床推广应用。

Objective To study the clinical value of addition and subtraction of Danggui Sini decoction in the treatment of patients with blood deficiency and cold coagulation type diabetic peripheral neuropathy(DPN).Methods 64 patients with blood deficiency and cold coagulation type DPN who were admitted to our hospital from June 2018 to November 2019 were selected as the research subjects.According to the computerized random blind selection method,they were divided into observation group and control group,with 32 cases in each group.The control group was treated with conventional western medicine(mecobalamin capsules,vitamin B1,folic acid tablets).The observation group was treated with addition and subtraction of Danggui Sini decoction on the basis of the control group.Compared the improvement of TCM syndrome score,Toronto clinical scoring system(TCSS)score,and conduction velocity of median nerve and common peroneal nerve before and after treatment between the two groups.Results After treatment,the scores of TCM syndrome and TCSS in the two groups were lower than before treatment,and the observation group was lower than the control group(P<0.05);the SNCV and MNCV of the median nerve and common peroneal nerve in the two groups were significantly higher than those before treatment,and the observation group were higher than the control group(P<0.05).Conclusion The application of addition and subtraction of Danggui Sini decoction in the treatment of diabetic peripheral neuropathy with blood deficiency and cold coagulation has significant effect,which can effectively improve the clinical symptoms and improve the nerve conduction speed of patients,high application value,worthy of clinical application.
