子宫内膜异位症致不孕的中医证候及用药规律研究

Research on TCM Syndrome and Medication Rule of Infertility Caused by Endometriosis

目的:通过频数分析、聚类分析等数据分析方法对现代文献进行研究,总结子宫内膜异位症致不孕的中医证候要素及用药规律。方法:通过中国期刊全网数据库(CNKI)、万方数据库、维普中文科技期刊数据库检索关于子宫内膜异位症致不孕的相关文献,提取证型、证候要素、方药等信息建立数据库,用SPSS22.0对证型、证素及药物进行频数分析,并对高频药物进行聚类分析。结果:检索到相关文献884篇,依据文献筛选原则最终纳入65篇。共提取病例3937例,涉及证型19个,最常见的是肾虚血瘀证(40.59%)和气滞血瘀证(20.22%);出现病性要素10个,最常见的是血瘀(52.07%)、气滞(15.86%)和肾虚(10.29%);病位要素5个,以肾(96.29%)最为常见。共涉及中药110种,出现频次较高的药物有菟丝子(54.20%)、丹参(54.13%)、当归(42.24%);对出现频次较高的25味中药进行聚类分析,得出6个药物组合。结论:子宫内膜异位症不孕临床常见证型是肾虚血瘀证,血瘀、肾分别是最常见的病性要素和病位要素,治疗内异症不孕临床常用补肾活血中药。

Objective:To study the modern literature through frequency analysis,cluster analysis and other data analysis methods,to summarize the TCM syndrome elements and medication rules of endometriosis causing infertility.Methods:Search the relevant literature on infertility caused by endometriosis through CNKI,Wanfang Database,and Weipu Chinese Science and Technology Journal Database,extract syndrome types,syndrome elements,prescriptions and other information to establish a database.Use SPSS22.0 to perform frequency analysis on syndrome types,syndrome elements and drugs,and perform cluster analysis on high-frequency drugs.Results:884 related documents were retrieved,and 65 were finally included in accordance with the principle of document selection.A total of 3937 cases were extracted,involving 19 syndrome types,the most common being kidney deficiency and blood stasis syndrome(40.59%)and qi stagnation and blood stasis syndrome(20.22%);10 pathogenic elements appeared,the most common being blood stasis(52.07%),stagnation of Qi(15.86%),and kidney deficiency(10.29%);there are 5 disease elements,with the kidney(96.29%)being the most common.A total of 110 kinds of traditional Chinese medicines are involved.The most frequently occurring medicines are Cuscuta(54.20%),Salvia miltiorrhiza(54.13%),and Angelica(42.24%);cluster analysis of 25 traditional Chinese medicines with a higher frequency shows 6 medicines combination.Conclusion:The common clinical syndrome of endometriosis infertility is kidney deficiency and blood stasis syndrome.Blood stasis and kidney are the most common disease elements and disease location elements,respectively.In the treatment of endometriosis infertility,traditional Chinese medicines for invigorating the kidney and promoting blood circulation are commonly used.
