宽体探测器CT“一站式”扫描的临床应用价值分析 被引量:1

摘要 目的分析宽体探测器电子计算机断层扫描(CT)行冠状动脉和头颈部CT血管成像(CTA)“一站式”扫描的临床应用价值。方法前瞻性选取100例2019年3月~2020年6月于我院进行宽体探测器CT检查的患者,根据扫描方法的不同分为A组(48例)、B组(52例)。A组行冠状动脉CTA检查(一次性注射对比剂),然后立即进行头颈部CTA检查;B组分别行冠状动脉CTA检查、头颈部CTA检查(注射2次对比剂)。观察两组冠状动脉CTA、头颈部CTA图像质量主观、客观评分及辐射剂量。结果两组冠状动脉CTA、头颈部CTA的图像质量主观评分,回旋支(LCX)、颈内动脉(ICA)、左前降支(LAD)、右冠状动脉(RCA)、大脑中动脉(MCA)、颈总动脉(ACC)的CT值,冠状动脉CTA、头颈部CTA的容积CT剂量指数(CTDIvol)、有效剂量(ED)比较差异无统计学意义(P>0.05)。B组冠状动脉及头颈部噪声(SD)值低于A组,冠状动脉及头颈部对比噪声比(CNR)高于A组(P<0.05)。结论宽体探测器CT“一站式”扫描的临床应用价值较高,可降低对比剂用量,简化患者检查流程,但检查过程中应注意调整检查位置以防止图片质量下降。 Objective To analyze the clinical application value of"one-stop"scanning of coronary artery computed tomography(CT)angiography and head and neck CTA with wide-body detector CT.Methods A total of 100 patients who underwent wide-body detector CT examination in our hospital from March 2019 to June 2020 were selected prospectively and divided into the group A(48 cases)and the group B(52 cases)according to the different scanning methods.The group A underwent coronary CTA(one-time injection of contrast agent),and then underwent head and neck CTA immediately.The group B received coronary CTA and head and neck CTA respectively(injected with contrast agent twice).The subjective and objective scores of image quality of coronary CTA and head and neck CTA as well as the radiation dose were observed in the two groups.Results There was no significant difference in subjective scores of image quality of coronary CTA and head and neck CTA;the CT values of circumflex artery(LCX),internal carotid artery(ICA),left anterior descending artery(LAD),right coronary artery(RCA),middle cerebral artery(MCA),artery common carotid(ACC);volume CT dose index(CTDIvol)and effective dose(ED)of coronary CTA and head and neck CTA between the two groups(P>0.05).The swishing(SD)values of coronary artery and head and neck in group B were lower than those of group A,and the contrast to noise ratio(CNR)of coronary arteryand head and neck was higher than that of group A(P<0.05).Conclusion"One-stop"scanning with wide-body detector CT had high clinical application value,which could reduce the dosage of contrast agent and simplify the examination process for patients.However,in the process of inspection,attention should be paid to adjusting the inspection position to prevent the decline of image quality.
