用多模式CT法判定超时间窗颅内前循环大血管闭塞患者是否适合进行血管内治疗的效果

目的:探讨用多模式CT法判定超时间窗颅内前循环大血管闭塞患者是否适合进行血管内治疗的效果。方法:回顾分析2019年1月至2020年10月期间在蚌埠医学院第二附属医院接受血管内治疗的65例颅内前循环大血管闭塞患者的临床资料。根据发病至接受血管穿刺时间的不同将其分为超时间窗组(发病至接受血管穿刺的时间为6~9 h,30例)和时间窗内组(发病至接受血管穿刺的时间在6 h以内,35例)。采用常规方法判定时间窗内组患者是否适合进行血管内治疗,采用多模式CT法判定超时间窗组患者是否适合进行血管内治疗。对两组患者中适合进行血管内治疗的患者均进行血管内治疗。然后比较两组患者的治疗效果及安全性。结果:治疗后,两组患者的血管再通率、异位栓塞、症状性出血的发生率、死亡率及其中mRS评分≤2分患者的占比相比,P>0.05。结论:用多模式CT法判定超时间窗颅内前循环大血管闭塞患者是否适合进行血管内治疗可取得较为理想的效果。

Objective:To explore the effect of multi-mode CT in determining whether patients with intracranial anterior circulation occlusion beyond time window are suitable for endovascular treatment.METHODS:The clinical data of 65 patients with intracranial anterior circulation great vessel occlusion who received endovascular treatment in the Second Affiliated Hospital of Bengbu Medical College from January 2019 to October 2020 were retrospectively analyzed.According to the different time from onset to receiving vascular puncture,the patients were divided into the extra-time window group(30 cases,the time from onset to receiving vascular puncture was 6-9 h)and the time-window group(35 cases,the time from onset to receiving vascular puncture was less than 6 h).Conventional methods were used to determine whether the patients in the time window group were suitable for endovascular treatment,and multi-mode CT was used to determine whether the patients in the extra-time window group were suitable for endovascular treatment.Patients in both groups who were suitable for endovascular therapy were treated with endovascular therapy.Then the treatment effect and safety of the two groups were compared.Results:After treatment,the rate of vascular recanculation,ectopia embolism,incidence of symptomatic bleeding,mortality and the proportion of patients with MRS score≤2 in 2 groups were compared,P>0.05.Conclusion:Multi-mode CT can be used to determine whether the patients with intracranial anterior circulation occlusion beyond time window is suitable for endovascular treatment.
