个案管理护理模式对2型糖尿病患者血糖控制及自我管理能力的影响

The Effect of Case Management Nursing Model on Blood Glucose Control and Self-management Ability of Type 2 Diabetic Patients

目的分析个案管理护理模式对2型糖尿病患者血糖控制效果和自我管理能力的影响。方法以随机抽样法抽取该院2018年5月-2020年4月180例2型糖尿病患者,随机实施分组,分成观察组和对照组,分别纳入90例患者,患者分别配合实施个案管理护理和常规护理。结果护理干预实施前,观察组和对照组患者的自我管理能力评估结果、血糖控制效果及生活质量评估结果,差异无统计学意义(P>0.05);护理干预实施后,相比对照组,观察组患者的自我管理能力有显著提升,差异有统计学意义(P<0.05);血糖控制效果改善程度更加显著,差异有统计学意义(P<0.05);生活质量有明显提高,差异有统计学意义(P<0.05)。结论个案管理护理干预能够显著提高2型糖尿病患者的自我管理能力,从而提高其血糖控制效果,改善其生活质量。

Objective To analyze the effect of case management nursing mode on the effect of blood sugar control and self-management ability in type 2 diabetes patients.Methods A random sampling method was used to select 180 patients with type 2 diabetes in the hospital from May 2018 to April 2020.They were randomly divided into observation group and control group.90 patients were enrolled,and the patients cooperated in the implementation of case management nursing and routine nursing.Results Before the implementation of nursing intervention,there was no significant difference in the evaluation results of self-management ability,blood sugar control effect and quality of life evaluation of patients in the observation group and the control group,the difference was not statistically significant(P>0.05);after the implementation of nursing intervention,compared with the control group,the observation group of the patient's self-management ability was significantly improved,the difference was statistically significant(P<0.05);the improvement of blood glucose control effect was more significant,the difference was statistically significant(P<0.05);The quality of life was significantly improved,the difference was statistically significant(P<0.05).Conclusion Case management nursing intervention can significantly improve the self-management ability of patients with type 2 diabetes,thereby improving their blood glucose control effect and improving their quality of life.
