基于“安全-支持-合作”模式ICU过渡期标准化护理方案的构建

Construction of standardized ICU transitional care scheme based on"safety,support,cooperation"model

目的构建ICU过渡期标准化护理方案。方法以ICU过渡期护理理论为依托,基于“安全-支持-合作”过渡期护理模式,构建ICU过渡期标准化护理方案概念框架。以Stetler证据应用模式为指导,按照识别、确认、决策3个步骤,从临床问题出发,筛选并整合最佳证据,确定方案初稿。选取13名专家,采用Delphi法对方案内容进行修订。结果两轮函询问卷回收率分别为92.31%和100%;专家权威系数分别为0.792和0.785;Kendall协调系数为0.172~0.500(P<0.05)。终版方案共计5个一级条目、15个二级条目和45个三级条目,各条目赋值均数为3.77~5.00,变异系数均小于0.25。结论ICU过渡期标准化护理方案科学性与可靠性较高,可用于规范ICU过渡期护理实践。

Objective To construct a standardized nursing scheme in ICU transitional period.Methods Based on the grounded theory of ICU transitional care and the"safety,support,cooperation"transitional care model,the conceptual framework of scheme was built.The best evidence was screened and integrated to help construct the initial scheme according to the first three steps of the Stetler model of research utilization,namely identification,confirmation and decision-making.Delphi expert correspondence were utilized to amend the initial scheme.Results Two rounds of expert enquiry were conducted within thirteen experts with the returning rates were 92.31%and 100%,the authority coefficients of the experts were 0.792 and 0.785 respectively.Kendall's conformance coefficient were between 0.179 and 0.500(P<0.05).The scheme consists of 5 first level items,15 second level items and 45 third level items,with mean value of each item was between 3.77 and 5.00.The coefficient of variation was less than 0.25.Conclusion The standardized ICU transitional care scheme is scientific and reliable that can be utilized to standardize nursing practice in ICU transitional period.
