扭曲蛋白、波形蛋白、血管生成拟态在三阴性乳腺癌组织中的表达及临床意义

Expression of Twist,vimentin and vasculogenic mimicry in triple-negative breast cancer tissues and its clinical significance

目的探讨扭曲蛋白(Twist)、波形蛋白(vimentin)、血管生成拟态(VM)在三阴性乳腺癌(TNBC)组织中的表达及临床意义。方法选择2013年1月至2015年12月南阳市中心医院收治的TNBC患者120例为研究对象,另选择同期因乳腺良性疾病行手术治疗的30例患者为对照。免疫组织化学法检测TNBC组织和乳腺良性病变组织中Twist、vimentin的表达水平,采用CD31和过碘酸-希夫液双染法检测TNBC癌组织及良性病变组织中VM的表达,并分析Twist、vimentin、VM的表达与TNBC患者临床病理学特征的关系。结果Twist、vimentin、VM在TNBC组织中阳性表达率分别为60.8%、25.0%、28.3%,在乳腺良性病变组织中的阳性表达率分别为13.3%、6.7%、0.0%(未检测到表达);Twist、vimentin、VM在TNBC组织中阳性表达率均高于乳腺良性病变组织(χ^(2)=21.675、4.807、10.991,P<0.05)。TNBC组织中Twist的表达与患者的淋巴结转移和临床分期有关(P<0.05),与患者的年龄、肿瘤直径、是否绝经无关(P>0.05);vimentin、VM的表达与肿瘤直径、淋巴结转移和临床分期有关(P<0.05),与患者的年龄、是否绝经无关(P>0.05)。TNBC患者癌组织中VM的表达与Twist、vimentin呈正相关(r=0.277、0.363,P<0.05);Twist的表达与vimentin呈正相关(r=0.384,P<0.05)。Twist、vimentin、VM阳性表达患者的生存时间均显著短于阴性表达患者(P<0.05)。Twist、vimentin、VM均呈阳性表达患者的生存率为29.4%,三者不全呈阳性表达患者的生存率为77.7%;Twist、vimentin、VM三者均呈阳性表达患者的生存率显著低于三者不全呈阳性表达患者(χ^(2)=44.620,P<0.05)。结论Twist、vimentin、VM在TNBC组织中高表达,Twist、vimentin、VM阳性表达提示TNBC患者预后不良。

Objective To investigate the expression and clinical significance of Twist,vimentin and vasculogenic mimicry(VM)in triple-negative breast cancer(TNBC)tissues.Methods A total of 120 patients with TNBC admitted to Nanyang Central Hospital from January 2013 to December 2015 were selected as the research objects,and another 30 patients with benign breast diseases were selected as the control.The expressions of Twist and vimentin in TNBC tissues and lesion tissues of benign breast diseases were detected by immunohistochemistry.The expression of VM in TNBC tissues and lesion tissues of benign breast disease was detected by CD31 and periodate Schiff solution double staining.The relationship between the expression of Twist,vimentin,VM and the clinical clinicopathological characteristics of TNBC patients was analyzed.Results The positive expression rates of Twist,vimentin and VM in TNBC tissues were 60.8%,25.0%and 28.3%respectively;the positive expression rates of Twist,vimentin and VM in lesion tissues of benign breast disease were 13.3%,6.7%and 0.0%(no expression was detected)respectively;the positive expression rates of Twist,vimentin and VM in TNBC tissues were higher than those in the lesion tissues of benign breast diseases(χ^(2)=21.675,4.807,10.991;P<0.05).The expression of Twist in TNBC tissues was associated with lymph node metastasis and clinical stage of patients(P<0.05);but not associated with the age,tumor diameter and menopause of patients(P>0.05).The expressions of vimentin and VM were correlated with the tumor diameter,lymph node metastasis and clinical stage(P<0.05);but not correlated with the age and menopause of patients(P>0.05).The expression of VM was positively correlated with Twist and vimentin(r=0.227,0.363;P<0.05);the expression of Twist was positively correlated with vimentin(r=0.384,P<0.05).The survival time of patients with Twist,vimentin and VM positive expression was significantly shorter than that with VM negative expression(P<0.05).The survival rate of patients with Twist,vimentin and VM positive expression was 29.4%,and the survival rate of patients with incomplete positive expression of Twist,vimentin and VM was 77.7%;the survival rate of patients with Twist,vimentin and VM positive expression was significantly lower than that with incomplete positive expression of Twist,vimentin and VM(χ^(2)=44.620,P<0.05).Conclusion Twist,vimentin and VM are highly expressed in TNBC tissue.The positive expression of Twist,vimentin and VM indicates poor prognosis of TNBC patients.
