祛湿化瘀解毒定痛汤治疗急性痛风性关节炎临床观察

Effect of Qushi Huayu Jiedu Dingtong Decoction in the Treatment of Acute Gouty Arthritis

目的研究祛湿化瘀解毒定痛汤治疗急性痛风性关节炎对患者关节功能及血清IL-1β、COX-2、CRP水平的影响。方法按照随机数字表法将2018年6月—2020年5月收治的138例急性痛风性关节炎患者,分为对照组(69例)和试验组(69例)。对照组给予依托考昔片治疗,试验组在对照组基础上联合祛湿化瘀解毒定痛汤治疗,2组均在治疗期间给予中医护理干预。7 d为一个疗程,2组患者均治疗2个疗程。对比2组治疗前后关节证候评分、关节功能及血清IL-1β、COX-2、CRP水平。结果与治疗前比,治疗后2组患者关节红肿、关节疼痛、关节活动受限等证候评分及血清IL-1β、COX-2、CRP水平均降低,且试验组低于对照组;治疗后试验组患者Lysholm评分及膝关节活动度均升高,且高于对照组(均P<0.05)。结论祛湿化瘀解毒定痛汤治疗急性痛风性关节炎可有效缓解患者临床症状,改善关节功能,同时降低患者血清IL-1β、COX-2、CRP水平,抑制机体炎症反应。

Objective To study the effects of Qushi Huayu Jiedu Dingtong decoction in the treatment of acute gouty arthritis on joint function and serum IL-1β,COX-2,CRP levels.Methods According to the random number table method,the 138 patients with acute gouty arthritis admitted to our hospital from June 2018 to May 2020 were divided into the control group(n=69)and the treatment group(n=69).The control group was given etoricoxib tablets,and the treatment group was treated with Qushi Huayu Jiedu Dingtong decoction on the basis of the control group.Both groups were given traditional Chinese medicine nursing intervention during the treatment.7 d was a course of treatment,both groups of patients were treated for 2 courses.Results Compared with before treatment,after treatment,the scores of syndromes such as joint swelling,joint pain,and limitation of joint movement,and serum IL-1β,COX-2,CRP levels of the two groups of patients decreased,and the treatment group was lower than the control group.After treatment,the Lysholm score and knee range of motion of the treatment group increased,and those of the treatment group were higher than those of the control group(all P<0.05).Conclusion Qushi Huayu Jiedu Dingtong decoction in the treatment of acute gouty arthritis could effectively relieve the clinical symptoms of patients with acute gouty arthritis,improve joint function,reduce serum IL-1β,COX-2,CRP levels,and inhibit inflammation.
