摘要
目的分析以磁共振成像(MRI)诊断膝关节损伤的临床价值及准确性。方法方便选取2019年6月—2020年5月该院骨科收诊膝关节损伤病例中94例患者,均行CT、MRI检查,以病理诊断结果作为“金标准”,统计检查结果,对比MRI、CT检查诊断膝关节损伤患者中骨损伤、韧带损伤、半月板损伤3种类型的准确度。结果该组共94例膝关节损伤患者,病理结果显示,骨损伤患者29例,韧带损伤患者31例,半月板损伤患者34例;CT检查结果显示,骨损伤患者28例,韧带损伤患者34例,半月板损伤患者32例;MRI检查结果显示,骨损伤患者29例,韧带损伤患者32例,半月板损伤患者33例。MRI诊断膝关节损伤中骨损伤的准确度(97.87%)、敏感度(96.55%)均相比CT检查较高,差异有统计学意义(χ^(2)=14.324、5.220,P<0.05);韧带损伤的准确度(98.94%)、敏感度(100.00%)均相比CT检查较高,差异有统计学意义(χ^(2)=8.902、4.276,P=0.003、0.039);半月板损伤的准确度(98.94%)、敏感度(97.06%)均相比CT检查较高,差异有统计学意义(χ^(2)=9.999、5.100,P=0.002、0.024)。结论MRI诊断膝关节损伤可较好展现膝关节损伤类型,对于患者的诊疗有重要意义。
Objective To analyze the clinical value and accuracy of magnetic resonance imaging(MRI)in the diagnosis of knee joint injuries.Methods Ninety-four patients were conveniently selected from the knee joint injury cases admitted by the orthopedic department of the hospital from June 2019 to May 2020.All of them underwent CT and MRI examinations.The pathological diagnosis results were used as the"gold standard".The results of statistical examinations were compared with MRI,the accuracy of CT examination in the diagnosis of bone injury,ligament injury,and meniscus injury in patients with knee joint injury.Results There were 94 patients with knee joint injury in this group.The pathological results showed that there were 29 patients with bone injury,31 patients with ligament injury,and 34 patients with meniscus injury.CT examination results showed that there were 28 patients with bone injury and 34 patients with ligament injury,32 patients with meniscus injury;MRI examination results showed that there were 29 patients with bone injury,32 patients with ligament injury,and 33 patients with meniscus injury.The accuracy(97.87%)and sensitivity(96.55%)of MRI in diagnosing bone injury in knee joint injuries are higher than CT examinations,and the difference was statistically significant(χ^(2)=14.324,5.220,P<0.05);the accuracy of ligament injuries(98.94%),sensitivity(100.00%),both were higher than CT examination,and the difference was statistically significant(χ^(2)=8.902,4.276,P=0.003,0.039);accuracy(98.94%)and sensitivity(97.06%)of meniscus injury,were higher than CT examination,and the difference was statistically significant(χ^(2)=9.999,5.100,P=0.002,0.024).Conclusion MRI diagnosis of knee joint injury can better show the type of knee joint injury,which is of great significance for the diagnosis and treatment of patients.
