皮下免疫联合沙美特罗替卡松粉吸入剂治疗中度儿童哮喘的疗效观察

Observation on the curative efficacy of subcutaneous immunotherapy combined with salmeterol fluticasone powder inhalation in treatment of children with moderate asthma

目的探讨皮下免疫联合沙美特罗替卡松粉吸入剂治疗中度儿童哮喘的疗效,为临床后续治疗提供可靠治疗方案。方法选取2017年1月-12月佛山市妇幼保健院门诊随访的中度支气管哮喘缓解期患儿60例为研究对象,按数字法随机分为观察组30例和对照组30例。对照组予沙美特罗替卡松粉吸入剂,早晚各1吸。观察组在对照组治疗基础上给予皮下免疫治疗,总疗程为1年。比较两组患儿哮喘发作次数、肺功能变化、临床控制率及每日沙美特罗替卡松粉吸入剂吸入剂量。结果治疗后随访1年,治疗后6个月、治疗后12个月,观察组患儿哮喘发作次数明显少于对照组,差异有统计学意义(均P<0.01)。治疗后6个月、12个月,观察组患儿第一秒用力呼气容积(FEV1)占预计值百分比、呼气峰流速(PEF)占预计值百分比显著高于对照组,差异有统计学意义(均P<0.01)。观察组患儿总有效率为93.33%,显著高于对照组为66.67%,差异有统计学意义(P<0.05)。治疗后6个月、治疗后12个月,观察组患儿每日沙美特罗替卡松粉吸入剂吸入剂量为均分布明显少于对照组,差异有统计学意义(均P<0.01)。结论皮下免疫联合沙美特罗替卡松粉吸入剂治疗中度儿童哮喘的疗效显著,具有很高的临床应用价值。

Objective To explore the curative efficacy of subcutaneous immunotherapy combined with salmeterol fluticasone powder inhalation in treatment of children with moderate asthma,provide a reliable therapeutic regimen for further treatment.Methods From January to December in 2017,60 children with moderate bronchial asthma in remission stage were selected from Outpatient Department of Foshan Municipal Maternal and Child Health Care Hospital,then they were divided into observation group and control group according to random number table,30 children in each group.The children in control group were treated by salmeterol fluticasone powder inhalation,twice a day;the children in observation group were treated by salmeterol fluticasone powder inhalation combined with subcutaneous immunotherapy,the treatment lasted for a year.The frequencies of asthma attack,pulmonary function changes,clinical control rates,and daily inhalation doses of salmeterol fluticasone powder were compared between the two groups.Results The frequencies of asthma attack at 6 and 12 months after treatment in observation group were statistically significantly less than those in control group(P<0.01).FEV1 and PEF at 6 and12 months after treatment in observation group were statistically significantly higher than those in control group(P<0.01).The total effective rate in observation group was 93.33%,which was statistically significantly higher than that in control group(66.67%)(P<0.05).The inhalation dose of salmeterol fluticasone powder at 6 and 12 months after treatment in observation group were statistically significantly less than those in control group(P<0.01).Conclusion The curative efficacy of subcutaneous immunotherapy combined with salmeterol fluticasone powder inhalation in treatment of children with moderate asthma is significant,the therapy has high clinical application value.
