抗苗勒管激素检测在评估女性卵巢储备功能中的价值 被引量:8

摘要 目的探讨抗苗勒管激素(AMH)在女性卵巢储备功能评估中的价值。方法选取2016年8月-2018年7月在江西省妇幼保健院妇科门科进行生育咨询的437例女性为研究对象,根据年龄分为研究组(35~45岁,216例)和对照组(25~34岁,221例),分析两组女性血清AMH与性激素结果,探讨AMH预测女性卵巢储备功能的价值。结果研究组女性AMH水平低于对照组,研究组卵泡刺激素(FSH)、基础雌激素(b E2)水平高于对照组,差异均有统计学意义(均P<0.05)。经AMH评估两组卵巢储备下降率,研究组(39.81%)高于对照组(18.10%),差异有统计学意义(P<0.05)。经FSH评估两组卵巢储备下降率,研究组(19.44%)高于对照组(16.29%),差异无统计学意义(P>0.05)。评估AMH和FSH对所有研究对象卵巢储备下降的检出情况,AMH的阳性检出率(28.83%)高于FSH的阳性检出率(17.85%),差异有统计学意义(P<0.05)。结论随着年龄增长,卵巢储备功能具有明显下降趋势;AMH在评价女性卵巢储备功能的灵敏度上要优于FSH,能更早反映妇女生育潜力的下降,值得临床推广。 Objective To explore the value of anti-Müllerian hormone(AMH)detection in evaluation of female ovarian reserve function.Methods From August 2016 to July 2018,437 women receiving fertility consultation in Gynecological Outpatient Department of Jiangxi Provincial Maternal and Child Health Care Hospital were selected and divided into study group(216 cases,35-45 years old)and control group(221 cases,25-34 years old)according to age.The results of serum AMH and sex hormones in the two groups were detected,the value of AMH in evaluation of female ovarian reserve function was analyzed.Results AMH level in study group was statistically significantly lower than that in control group,the levels of follicle-stimulating hormone(FSH)and basic estradiol(bE_(2))in study group were statistically significantly higher than those in control group(P<0.05).The reduction rate of ovarian reserve function evaluated by AMH in study group(39.81%)was statistically significantly higher than that in control group(18.10%)(P<0.05).The reduction rate of ovarian reserve function evaluated by FSH in study group(19.44%)was statistically significantly higher than that in control group(16.29%)(P>0.05).The positive detection rate of AMH for reduction of ovarian reserve function(28.83%)was statistically significantly higher than that of FSH(17.85%)(P<0.05).Conclusion Ovarian reserve function decreases significantly with age,the sensitivity of AMH in evaluation of female ovarian reserve function is higher than that of FSH,AMH can early reflect the reduction of female fertility potential,which is worthy of clinical promotion.
