左甲状腺素钠对桥本甲状腺炎患者的治疗效果及对TGAb水平的影响

The therapeutic effect of levothyroxine sodium on Hashimoto's thyroiditis and its effect on TGAb level

目的:使用左甲状腺素钠对桥本甲状腺炎患者进行治疗,观察效果。方法:选取2017年5月~2019年5月收治的桥本甲状腺炎患者126例,随机分为左甲状腺素钠组和哂酵母组,各63例。哂酵母组使用硒酵母片对患者进行治疗,左甲状腺素钠组使用哂酵母片联合左甲状腺素钠片对患者进行治疗。化学发光免疫分析法测定TPOab、TGab水平,酶联免疫吸附试验法检测血清中IL-12、IL-10水平,使用生活质量评价量表对患者感知功能、社会功能、生理功能、情绪功能进行评分,对比两组患者的治疗情况,列出临床疗效对比表,并详细记用药后产生的不良反应。结果:左甲状腺素钠组TPOab、TGab、IL-12、FT3水平低于哂酵母组,IL-10、FT4、THS水平高于哂酵母组,差异有统计学意义(P<0.05)。治疗后左甲状腺素钠组感知功能、社会功能、生理功能、情绪功能评分高于哂酵母组,差异有统计学意义(P<0.05)。左甲状腺素钠组总治疗率高于哂酵母组总治疗率,差异有统计学意义(P<0.05)。哂酵母组总不良发生率高于左甲状腺素钠组总不良发生率,差异有统计学意义(P<0.05)。结论:使用左甲状腺素钠对桥本甲状腺炎患者进行治疗,减少甲状腺功能的破坏,提升甲状腺素,降低患者免疫反应,提高患者生活质量,能够有效调控TGAb水平。

Objective Left thyroxine sodium was used to treat Hashimoto's thyroiditis,to observe the effect.Method A total of 126 patients with hashimoto thyroiditis admitted to our hospital from May 2017 to May 2019 were selected and randomly divided into the levothyroxine sodium group and the yeast group(63 cases in each group).Patients were treated with selenium yeast tablets in the yeast group and selenium yeast tablets combined with levothyroxine sodium tablets in the levothyroxine sodium group.Chemiluminescence immunoassay,TPOab TGab level,enzyme-linked immunosorbent method to detect the serum level of IL-12,IL-10,using life quality evaluation scale for patients with cognitive function,social function,physiological function,emotional function score,compared two groups of patients with treatment,clinical curative effect comparison list,and detailed record of the medicine adverse reaction.Results The levels of TPOAb,TGAb,IL-12,FT3 in levothyroxine sodium group were lower than those in the yeast group,and the levels of IL-10,FT4,THS in levothyroxine sodiumgroup were nigher than those in the yeast group(P<0.05).After treatment,the scores of sensory function,social function,physiological function and emotional function in levothyroxine sodium group were higher than those in yeast group(P<0.05).The total treatment rate of levothyroxine sodium group was higher than that of yeast group(P<0.05).The incidence of total adverse reactions in the yeast group was higher than that in the sodium levothyroxine group(P<0.05).Conclusion The treatment of Hashimoto's thyroiditis with levothyroxine sodium can reduce the damage of thyroid function,improve the level of thyroxine,reduce the immune response of patients,improve the quality of life of patients,and effectively regulate the level of TGAb.
