初产妇母乳喂养认知行为干预的临床效果研究

Study on the clinical effect of cognitive behavior intervention on breast-feeding of primiparas

目的探讨认知行为干预在初产妇母乳喂养中的应用效果。方法选取2020年6月至12月于中国医科大学附属盛京医院分娩的初产妇162例为研究对象,随机分为对照组与干预组,每组81例。对照组采用常规护理干预,干预组采用认知行为干预,对比两组初产妇初乳分泌时间、产后乳房充盈时间、24h母乳喂养次数、出院后第7d、第12d、第30d母乳喂养情况和母乳喂养满意度等。结果干预组产后乳房充盈时间快于对照组,初乳分泌时间早于对照组,产后24h母乳喂养次数较对照组更多,差异均有统计学意义(t值分别为14.12、13.21、14.43,均P<0.05);干预组产妇出院后第12d、第30d纯母乳喂养率均显著高于对照组(χ^(2)值分别为7.12、7.98,均P<0.05),两组出院后第7d纯母乳喂养率无统计学差异(P>0.05);干预组中产妇出院后第7d、第12d、第30d的母乳喂养满意度均显著高于对照组,差异有统计学意义(t值分别为19.66、69.75、50.54,均P<0.05);干预组30d后的乳房胀痛以及乳房炎症发生率均显著低于对照组(χ^(2)值分别为6.76、4.34,均P<0.05)。结论认知行为干预有助于促进初产妇乳汁分泌,提高纯母乳喂养率和母乳喂养满意度,值得临床借鉴使用。

Objective To explore the application effect of cognitive behavior intervention on breast-feeding of primiparas.Methods A total of 162 primiparas admitted in Shengjing Hospital Affiliated to China Medical University from June 2020 to December 2020 were selected as the study objects and randomly divided into control group and intervention group.The control group was given routine nursing intervention, while the intervention group was given cognitive behavior intervention nursing.The colostrum secretion time, postpartum breast filling time and 24-hour breast-feeding times of the two groups were compared.The breast-feeding ratio and breast-feeding satisfaction on the 7 th, 12 th and 30 th day after discharge were analyzed.Results The postpartum breast filling time and colostrum secretion time and in intervention group were shorter than those in the control group, while 24-hour breast-feeding times was significantly more than control group(t values were 14.12,13.21 and 14.43,respectively, all P<0.05).The breast-feeding ratios on the 12 th and 30 th day after discharge in the intervention group were significantly higher than those in the control group(χ^(2) values were 7.12 and 7.98,respectively, all P<0.05),but no significant difference was found in the breast-feeding ratio on the 7 th day after discharge(P>0.05).In addition, the breast-feeding satisfaction on the 7 th, 12 th and 30 th day after discharge in intervention group were significantly higher than those in control group, differences were statistically significant(t values were 19.66,69.75 and 50.54,respectively, all P<0.05).The incidence of breast tenderness, breast inflammation in the intervention group were significantly lower than those in the control group, the differences were statistically significant(χ^(2) values were 6.76 and 4.34,respectively, both P<0.05).Conclusion Cognitive behavioral intervention can promote the galactosis of primiparas, improve the rate of exclusive breastfeeding and satisfaction of breastfeeding, which is worthy of clinical reference and use.
