应用消融电极对甲状旁腺功能保护的初步探讨

摘要 目的初步探讨应用消融电极对甲状旁腺功能的保护作用。方法选择2019年11月至2020年4月本科室收治的行甲状腺手术的患者248例,随机分为A组和B组各124例,A组使用超声刀进行手术,B组使用消融电极进行手术。比较两组患者的手术效果及术后并发症发生情况。结果A组与B组在手术时间、术中出血量、术后引流量以及术后住院时间方面差异无统计学意义(P>0.05)。A组患者术后有4例出现声音嘶哑,3例伴饮水呛咳,5例手足麻木抽搐;B组患者术后有2例出现声音嘶哑,无伴饮水呛咳,2例手足麻木感,A组与B组术后并发症总发生率比较差异有统计学意义(P<0.05)。结论消融电极与超声刀的手术效果相近,但消融电极对于甲状旁腺的保护更具有优势,可以减少术后甲状旁腺功能的损伤,操作简便,值得推广。 Objective To investigate the protective effect of ablation electrode on parathyroid function.Methods A total of 248 patients with thyroid surgery in our department from November 2019 to April 2020 were selected and randomly divided into group A and group B,with 124 cases in each group.Group A was operated with ultrasonic scalpel and group B was operated with ablation electrode.The operation effect and postoperative complications of the two groups were compared.Results There was no significant difference in operation time,intraoperative blood loss,postoperative drainage volume and postoperative hospital stay between group A and group B(P>0.05).In group A,there were 4 cases with postoperative hoarseness,3 cases with drinking water cough,5 cases with hand and foot numbness and convulsion.In group B,there were 2 cases with postoperative hoarseness,no drinking water cough,and 2 cases with hand and foot numbness.The total incidence of postoperative complications between group A and group B was statistically different(P<0.05).Conclusion The effect of ablation electrode is similar to that of ultrasonic scalpel,and the ablation electrode has more advantages for the protection of parathyroid gland,which can reduce the damage of parathyroid function after operation,and it is easy to operate and worth promoting.
