硝普钠与新活素治疗老年顽固性心衰的效果对比

Effect comparison of sodium nitroprusside and Xinhuosu in the treatment of elderly patients with intractable heart failure

目的探讨老年顽固性心力衰竭(心衰)患者应用硝普钠与冻干重组人脑利钠肽(商品名:新活素)治疗的疗效。方法100例老年顽固性心衰患者,随机分为对照组与观察组,各50例。对照组应用硝普钠治疗,观察组应用新活素治疗。对比两组治疗效果、心功能指标、不良反应发生情况。结果观察组治疗总有效率为98%,高于对照组的82%,差异有统计学意义(P<0.05)。观察组B型脑钠肽(BNP)为(136.84±21.79)pg/ml、左室射血分数(LVEF)为(42.01±3.74)%,优于对照组的(177.04±31.69)pg/ml、(39.79±3.01)%,差异有统计学意义(P<0.05)。两组左室舒张末期内径(LVEDd)对比差异无统计学意义(P>0.05)。观察组不良反应发生率为4%,低于对照组的16%,差异有统计学意义(P<0.05)。结论老年顽固性心衰应用新活素治疗,可提升治疗效果,明显改善患者心功能,不良反应少,建议临床应用。

Objective To discuss the efficacy of sodium nitroprusside and lyophilized recombinant human brain natriuretic peptide(trade name:Xinhuosu)in the treatment of elderly patients with intractable heart failure.Methods A total of 100 elderly patients with intractable heart failure were randomly divided into control group and observation group,with 50 cases in each group.The control group was treated with sodium nitroprusside,and the observation group was treated with Xinhuosu.The therapeutic effect,cardiac function indicators,and occurrence of adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 98%,which was higher than 82%of the control group,and the difference was statistically significant(P<0.05).The B-type natriuretic peptide(BNP)(136.84±21.79)pg/ml and left ventricular ejection fraction(LVEF)(42.01±3.74)%of the observation group were better than(177.04±31.69)pg/ml and(39.79±3.01)%of the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in left ventricular end diastolic diameter(LVEDd)between the two groups(P>0.05).The incidence of adverse reactions of the observation group was 4%,which was lower than 16%of the control group,and the difference was statistically significant(P<0.05).Conclusion In the treatment of elderly intractable heart failure,the application of Xinhuosu can improve the therapeutic effect,and significantly improve the cardiac function with less adverse reactions.It is recommended for clinical application.
