彩超引导下经皮球囊扩张术治疗动静脉内瘘狭窄的中期效果

Mid-term effect of color ultrasound-guided percutaneous transluminal angioplasty in the treatment of arteriovenous fistula stenosis

目的探讨彩超引导下经皮球囊扩张术(PTA)治疗动静脉内瘘(AVF)狭窄的中期效果。方法选取2017年1月~2018年6月江西省上饶市人民医院收治的60例AVF狭窄患者作为研究对象,采用随机数字表法分为两组,每组各30例。观察组采用PTA治疗,对照组采用经皮血栓切除术。记录两组手术前后临床指标,并统计两组不良事件发生情况及术后18个月内瘘通畅情况。结果两组术后内瘘狭窄处内径大于术前,内瘘自然流量、透析血流量均高于术前,且观察组高于对照组,差异有统计学意义(P<0.05)。观察组术后不良事件总发生率低于对照组,术后18个月内瘘通畅率高于对照组,差异有统计学意义(P<0.05)。结论AVF狭窄患者采用PTA治疗可有效改善临床指标,且具有一定的安全性。

Objective To investigate the mid-term effect of color ultrasound-guided percutaneous transluminal angioplasty(PTA)in the treatment of arteriovenous fistula(AVF)stenosis.Methods A total of 60 cases of patients with AVF stenosis treated in Shangrao People′s Hospital of Jiangxi Province from January 2017 to June 2018 were selected as the research objects,and they were divided into two groups by the random number table method,with 30 cases in each group.The observation group was treated with PTA,and the control group was treated with percutaneous thrombectomy.The clinical indexes before and after operation of the two groups were recorded,and the occurrence of adverse events and the patency of internal fistula at 18 months after operation were counted.Results The internal diameter of the internal fistula stenosis in both groups after operation was larger than that before operation,the natural flow of the internal fistula,and the blood flow of dialysis were all higher than those before operation,and those in the observation group were higher than those in the control group,with statistical significant differences(P<0.05).The total incidence of postoperative adverse events in the observation group was higher than that in the control group,the patency rate of internal fistula at 18 months after operation was higher than that in the control group,with statistical significant differences(P<0.05).Conclusion PTA in treatment of patients with AVF stenosis can improve the clinical indexes,which has certain safety.
