左侧卧位在腹腔镜肝右后叶切除术中的应用

Application of left decubitus position in laparoscopic hepatic right posterior lobectomy

目的:探讨左侧卧位在腹腔镜肝右后叶切除术中的应用效果。方法:将2020年1月至12月收治的59例肝右后叶肿瘤患者随机分为两组,两组患者均行腹腔镜手术,其中36例采用左侧卧位(左侧位组),23例采用人字位(人字位组),对比两组手术时间、出血量、术中肝门阻断时间、术中手术床调整频次。结果:左侧位组手术时间、出血量、肝门阻断时间、术中手术床调整频次均优于人字位组,差异有统计学意义(P<0.05)。结论:左侧卧位在腹腔镜肝右后叶切除术中能更好地暴露术野,方便术者操作,从而缩短手术时间,减少术中出血量,缩短肝门阻断时间,减少术中手术床的调整频次,具有临床实用价值。

Objective:To investigate the clinical effect of left decubitus position in laparoscopic hepatic right posterior lobectomy.Methods:From Jan.2020 to Dec.2020,59 patients with hepatic right posterior lobe tumor were randomly divided into two groups.All the operations were performed by laparoscopy.36 cases were operated in left lateral position,23 cases were operated in herringbone position.The operative time,amount of blood loss,intraoperative time of hepatic portal occlusion and frequency of intraoperative operating table adjustment were compared between the two groups.Results:The operative time,blood loss,time of hepatic portal occlusion and frequency of intraoperative operating table adjustment in left lateral position group were better than those in herringbone position group,the differences were statistically significant(P<0.05).Conclusions:The left lateral position in laparoscopic hepatic right posterior lobectomy can obtain better operative field exposure and convenient operation,so as to reduce the operation time,intraoperative blood loss,hepatic portal occlusion time and frequency of intraoperative operating table adjustment.This procedure is of clinical practical value.
